4     Between lines 13 and 14, insert:
5     Section 1.  Paragraphs (e), (f), (g), and (h) are added to
6subsection (6) of section 550.2415, Florida Statutes, to read:
7     550.2415  Racing of animals under certain conditions
8prohibited; penalties; exceptions.--
9     (6)
10     (e)  The division shall maintain accurate records and
11statistics regarding injuries incurred by greyhounds that race
12in this state. The division shall adopt rules requiring the
13reporting of injuries incurred by greyhounds while racing in
14this state, including schooling races. Such reports must
15include:
16     1.  The greyhound's registered name and right and left ear
17tattoo numbers.
18     2.  The name, business address, and telephone number of the
19greyhound owner, trainer, and kennel operator.
20     3.  The color, weight, and sex of the greyhound.
21     4.  The specific type of injury, the cause of the injury,
22the estimated recovery time, and the location of the injury on
23the greyhound.
24     5.  Where the injury occurred, whether on a racing track or
25in another area.
26     6.  If the injury occurred while the greyhound was racing,
27the racetrack where the injury occurred; the distance, grade,
28race, and post position when the injury occurred; and the
29weather conditions, time, temperature, and track condition at
30the time of the injury.
31     7.  A certification by the racetrack veterinarian that the
32form is correct.
33     (f)  The division shall maintain accurate records and
34statistics regarding the disposition of greyhounds that
35participate in racing in this state. The division shall adopt
36rules requiring the reporting of the disposition of greyhounds
37that race in this state, including schooling races. As used in
38the reporting requirement, the term "disposition" means death,
39transfer to another jurisdiction, retirement, adoption, sale, or
40donation for medical research or another purpose. Such reports
41must include:
42     1.  The greyhound's registered name and right and left ear
43tattoo numbers; the name, business address, and telephone number
44of the greyhound owner, trainer, and kennel operator; and the
45name and address of the race track where the greyhound last
46raced prior to disposition.
47     2.  If the greyhound was transferred to another track, the
48name and address of the track that received the greyhound and
49the name, business address, telephone number, and driver's
50license number and state of issuance of the person who received
51the greyhound on behalf of that track.
52     3.  If the greyhound was retired for breeding, the name and
53address of the facility that received the greyhound and the
54name, business address, telephone number, and driver's license
55number and state of issuance of the person who received the
56greyhound on behalf of that facility.
57     4.  If the greyhound was adopted or placed for adoption,
58the name and address of the person that received the greyhound
59and, if applicable, the name, business address, telephone
60number, and driver's license number and state of issuance of the
61person who received the greyhound on behalf of the adoption
62facility.
63     5.  If the greyhound was euthanized, the name, address,
64professional title, professional affiliation of the person
65performing the euthanasia, method of euthanasia, and reason the
66greyhound was euthanized rather than adopted.
67     6.  If the greyhound was sold or donated, the name of the
68person to whom the greyhound was sold or donated, and if
69donated, the name, business address, telephone number, and
70driver's license number and state of issuance of the person who
71received the greyhound on behalf of the donee.
72     7.  If the disposition of the greyhound does not fit into
73any of the above categories, the name of the person to whom the
74greyhound was transferred, and the name, business address,
75telephone number, and driver's license number and state of
76issuance of the person who received the greyhound.
77     8.  Certification by the owner, trainer, and kennel
78operator that the disposition forms are correct.
79     (g)  The division shall maintain injury and disposition
80records for 7 years.
81     (h)  In addition to other penalties imposed by law, a
82person who knowingly makes a false statement on an injury or
83disposition form commits a misdemeanor of the first degree,
84punishable as provided in s. 775.082 or s. 775.083. A person who
85knowingly makes a false statement on an injury or disposition
86form on a second or subsequent occasion commits a felony of the
87third degree, punishable as provided in s. 775.082, s. 775.083,
88or s. 775.084.

93F.S.; requiring the Division of Pari-mutuel Wagering in the
94Department of Business and Professional Regulation to maintain
95certain records regarding injuries and the disposition of
96greyhounds; providing guidelines and requirements for injury and
97disposition report forms; providing for the adoption of rules;
98providing penalties; amending s. 849.086, F.S.;
