     (2)(a) Upon request, such information shall may be

disclosed to:

     1. Another governmental entity only if disclosure is

11

necessary for the entity to perform its duties and

12

responsibilities under the Florida Kidcare program; and shall be

13

disclosed to

     2. The Department of Revenue for purposes of administering

15

the state Title IV-D program; or. The receiving governmental

16

entity must maintain the confidential and exempt status of such

17

information. Furthermore, such information may not be released

18

to

19

     3. Any person who has without the written consent of the

20

program applicant.

     (b) This section does not prohibit an enrollee's legal

22

guardian from obtaining confirmation of coverage, dates of

23

coverage, name of the enrollee's health plan, and amount of

24

premium being paid.
