10 | (f) Notwithstanding s. 320.023, the application form for |
11 | motor vehicle registration and renewal of registration must |
12 | include language permitting a voluntary contribution of $1 per |
13 | applicant to aid the homeless. Contributions made pursuant to |
14 | this paragraph shall be deposited into the Grants and Donations |
15 | Trust Fund of the Department of Children and Family Services and |
16 | used by the State Office on Homelessness to supplement grants |
17 | made under s. 420.622(4) and (5), provide information to the |
18 | public about homelessness in the state, and provide literature |
19 | for homeless persons seeking assistance. |

21 | For the purpose of applying the service charge provided in s. |
22 | 215.20, contributions received under this subsection are not |
23 | income of a revenue nature. |
24 | Section 2. Subsection (6) of section 322.08, Florida |
25 | Statutes, is amended to read: |
26 | 322.08 Application for license.-- |
27 | (6) The application form for a driver's license or |
28 | duplicate thereof shall include language permitting the |
29 | following: |
30 | (a) A voluntary contribution of $5 per applicant, which |
31 | contribution shall be transferred into the Election Campaign |
32 | Financing Trust Fund. |
33 | (b) A voluntary contribution of $1 per applicant, which |
34 | contribution shall be deposited into the Florida Organ and |
35 | Tissue Donor Education and Procurement Trust Fund for organ and |
36 | tissue donor education and for maintaining the organ and tissue |
37 | donor registry. |
38 | (c) A voluntary contribution of $1 per applicant, which |
39 | contribution shall be distributed to the Florida Council of the |
40 | Blind. |
41 | (d) A voluntary contribution of $2 per applicant, which |
42 | shall be distributed to the Hearing Research Institute, |
43 | Incorporated. |
44 | (e) A voluntary contribution of $1 per applicant, which |
45 | shall be distributed to the Juvenile Diabetes Foundation |
46 | International. |
47 | (f) A voluntary contribution of $1 per applicant, which |
48 | shall be distributed to the Children's Hearing Help Fund. |
49 | (g) Notwithstanding s. 322.081, a voluntary contribution |
50 | of $1 per applicant to aid the homeless. Contributions made |
51 | pursuant to this paragraph shall be deposited into the Grants |
52 | and Donations Trust Fund of the Department of Children and |
53 | Family Services and used by the State Office on Homelessness to |
54 | supplement grants made under s. 420.622(4) and (5), provide |
55 | information to the public about homelessness in the state, and |
56 | provide literature for homeless persons seeking assistance. |

58 | A statement providing an explanation of the purpose of the trust |
59 | funds shall also be included. For the purpose of applying the |
60 | service charge provided in s. 215.20, contributions received |
61 | under paragraphs (c), (d), (e), and (f), and (g) and under s. |
62 | 322.18(9)(a) are not income of a revenue nature. |
63 | Section 3. Subsection (9) of section 322.18, Florida |
64 | Statutes, is amended to read: |
65 | 322.18 Original applications, licenses, and renewals; |
66 | expiration of licenses; delinquent licenses.-- |
67 | (9)(a) The application form for a renewal issuance or |
68 | renewal extension shall include language permitting a voluntary |
69 | contribution of $1 per applicant, to be quarterly distributed by |
70 | the department to Prevent Blindness Florida, a not-for-profit |
71 | organization, to prevent blindness and preserve the sight of the |
72 | residents of this state. A statement providing an explanation of |
73 | the purpose of the funds shall be included with the application |
74 | form. |
75 | (b) Prior to the department distributing the funds |
76 | collected pursuant to this paragraph (a), Prevent Blindness |
77 | Florida must submit a report to the department that identifies |
78 | how such funds were used during the preceding year. |
79 | (b) The application form for a renewal issuance or renewal |
80 | extension shall include language permitting a voluntary |
81 | contribution of $1 per applicant to aid the homeless. |
82 | Contributions made pursuant to this paragraph shall be deposited |
