3     Amendment (with title amendment)
4     Between lines 34 and 35, insert:
5     Section 1.  Paragraph (f) is added to subsection (16) of
6section 320.02, Florida Statutes, to read:
7     320.02  Registration required; application for
8registration; forms.--

10     (f)  Notwithstanding s. 320.023, the application form for
11motor vehicle registration and renewal of registration must
12include language permitting a voluntary contribution of $1 per
13applicant to aid the homeless. Contributions made pursuant to
14this paragraph shall be deposited into the Grants and Donations
15Trust Fund of the Department of Children and Family Services and
16used by the State Office on Homelessness to supplement grants
17made under s. 420.622(4) and (5), provide information to the
18public about homelessness in the state, and provide literature
19for homeless persons seeking assistance.

21For the purpose of applying the service charge provided in s.
22215.20, contributions received under this subsection are not
23income of a revenue nature.
24     Section 2.  Subsection (6) of section 322.08, Florida
25Statutes, is amended to read:
26     322.08  Application for license.--
27     (6)  The application form for a driver's license or
28duplicate thereof shall include language permitting the
29following:
30     (a)  A voluntary contribution of $5 per applicant, which
31contribution shall be transferred into the Election Campaign
32Financing Trust Fund.
33     (b)  A voluntary contribution of $1 per applicant, which
34contribution shall be deposited into the Florida Organ and
35Tissue Donor Education and Procurement Trust Fund for organ and
36tissue donor education and for maintaining the organ and tissue
37donor registry.
38     (c)  A voluntary contribution of $1 per applicant, which
39contribution shall be distributed to the Florida Council of the
40Blind.
41     (d)  A voluntary contribution of $2 per applicant, which
42shall be distributed to the Hearing Research Institute,
43Incorporated.
44     (e)  A voluntary contribution of $1 per applicant, which
45shall be distributed to the Juvenile Diabetes Foundation
46International.
47     (f)  A voluntary contribution of $1 per applicant, which
48shall be distributed to the Children's Hearing Help Fund.
49     (g)  Notwithstanding s. 322.081, a voluntary contribution
50of $1 per applicant to aid the homeless. Contributions made
51pursuant to this paragraph shall be deposited into the Grants
52and Donations Trust Fund of the Department of Children and
53Family Services and used by the State Office on Homelessness to
54supplement grants made under s. 420.622(4) and (5), provide
55information to the public about homelessness in the state, and
56provide literature for homeless persons seeking assistance.

58A statement providing an explanation of the purpose of the trust
59funds shall also be included. For the purpose of applying the
60service charge provided in s. 215.20, contributions received
61under paragraphs (c), (d), (e), and (f), and (g) and under s.
62322.18(9)(a) are not income of a revenue nature.
63     Section 3.  Subsection (9) of section 322.18, Florida
64Statutes, is amended to read:
65     322.18  Original applications, licenses, and renewals;
66expiration of licenses; delinquent licenses.--
67     (9)(a)  The application form for a renewal issuance or
68renewal extension shall include language permitting a voluntary
69contribution of $1 per applicant, to be quarterly distributed by
70the department to Prevent Blindness Florida, a not-for-profit
71organization, to prevent blindness and preserve the sight of the
72residents of this state. A statement providing an explanation of
73the purpose of the funds shall be included with the application
74form.
75     (b)  Prior to the department distributing the funds
76collected pursuant to this paragraph (a), Prevent Blindness
77Florida must submit a report to the department that identifies
78how such funds were used during the preceding year.
79     (b)  The application form for a renewal issuance or renewal
80extension shall include language permitting a voluntary
81contribution of $1 per applicant to aid the homeless.
82Contributions made pursuant to this paragraph shall be deposited
83into the Grants and Donations Trust Fund of the Department of
84Children and Family Services and used by the State Office on
85Homelessness to supplement grants made under s. 420.622(4) and
86(5), provide information to the public about homelessness in the
87state, and provide literature for homeless persons seeking
88assistance.

101include an option to make a voluntary contribution to aid the
102homeless; providing that voluntary contributions for the
103homeless are not income of a revenue nature for the purpose of
104applying certain service charges; providing for such
105contributions to be deposited into the Grants and Donations
106Trust Fund of the Department of Children and Family Services and
107used by the State Office on Homelessness for certain purposes;
108creating s. 414.161,
