15 | Be It Enacted by the Legislature of the State of Florida: |
16 |

17 | Section 1. Paragraph (f) is added to subsection (16) of |
18 | section 320.02, Florida Statutes, to read: |
19 | 320.02 Registration required; application for |
20 | registration; forms.-- |
21 | (16) |
22 | (f) The application form for motor vehicle registration |
23 | and renewal of registration must include language permitting a |
24 | voluntary contribution of $1 to Family First. Such contributions |
25 | must be transferred by the department each month to Family |
26 | First, a nonprofit organization. |

28 | For the purpose of applying the service charge provided in s. |
29 | 215.20, contributions received under this subsection are not |
30 | income of a revenue nature. |
31 | Section 2. Subsection (6) of section 322.08, Florida |
32 | Statutes, is amended to read: |
33 | 322.08 Application for license.-- |
34 | (6) The application form for a driver's license or |
35 | duplicate thereof shall include language permitting the |
36 | following: |
37 | (a) A voluntary contribution of $5 per applicant, which |
38 | contribution shall be transferred into the Election Campaign |
39 | Financing Trust Fund. |
40 | (b) A voluntary contribution of $1 per applicant, which |
41 | contribution shall be deposited into the Florida Organ and |
42 | Tissue Donor Education and Procurement Trust Fund for organ and |
43 | tissue donor education and for maintaining the organ and tissue |
44 | donor registry. |
45 | (c) A voluntary contribution of $1 per applicant, which |
46 | contribution shall be distributed to the Florida Council of the |
47 | Blind. |
48 | (d) A voluntary contribution of $2 per applicant, which |
49 | shall be distributed to the Hearing Research Institute, |
50 | Incorporated. |
51 | (e) A voluntary contribution of $1 per applicant, which |
52 | shall be distributed to the Juvenile Diabetes Foundation |
53 | International. |
54 | (f) A voluntary contribution of $1 per applicant, which |
55 | shall be distributed to the Children's Hearing Help Fund. |
56 | (g) A voluntary contribution of $1 per applicant, which |
57 | contribution shall be distributed to Family First, a nonprofit |
58 | organization. |

60 | A statement providing an explanation of the purpose of the trust |
61 | funds shall also be included. For the purpose of applying the |
62 | service charge provided in s. 215.20, contributions received |
63 | under paragraphs (c), (d), (e), and (f), and (g) and under s. |
64 | 322.18(9)(a) are not income of a revenue nature. |
65 | Section 3. Subsection (9) of section 322.18, Florida |
66 | Statutes, is amended to read: |
67 | 322.18 Original applications, licenses, and renewals; |
68 | expiration of licenses; delinquent licenses.-- |
69 | (9)(a) The application form for a renewal issuance or |
70 | renewal extension shall include language permitting a voluntary |
71 | contribution of $1 per applicant, to be quarterly distributed by |
72 | the department to Prevent Blindness Florida, a not-for-profit |
73 | organization, to prevent blindness and preserve the sight of the |
74 | residents of this state. A statement providing an explanation of |
75 | the purpose of the funds shall be included with the application |
76 | form. |
77 | (b) Prior to the department distributing the funds |
78 | collected pursuant to this paragraph (a), Prevent Blindness |
79 | Florida must submit a report to the department that identifies |
80 | how such funds were used during the preceding year. |
81 | (b) The application form for a renewal issuance or renewal |
82 | extension shall include language permitting a voluntary |
83 | contribution of $1 per applicant to Family First. |
84 | Section 4. This act shall take effect October 1, 2008. |
