3. An assessment of the capacity of service providers and

their ability to meet the needs of the residents in the
10
communities where residents will likely reside after they leave
11
the institution.

4. A plan for service provider recruitment, development,
13
and training, as needed, to ensure that adequate services are
14
available to residents as they make the transition into the
15
community.

5. A plan for monitoring and ensuring safety and service
17
quality for residents after they have left the institution.

6. A process for developing a support plan that includes
19
consultation with the residents and their guardians for
20
providing services and other residential placement plans for
21
residents after they leave the institution.

7. A plan for adjusting employee staffing levels during
23
the census reduction or transition to closure to ensure the
24
safety of and quality of care for residents and assistance for
25
employees seeking new employment.

8. A complete timeline for closure of or census reduction
27
at the institution which includes a financial plan for the
28
closure or census reduction and the projected savings associated
29
with the closure or census reduction.

9. A communications plan to keep residents, guardians of
31
residents, designated family members, employees, and designees
32
informed of the progress of the closure or census reduction.
33
10. The responsibility of each state agency involved in
34
the closure of or census reduction at the institution.
35
11. A plan for closure activities and the disposition of
36
property of the physical plant of the institution.
