     (d) In addition to the requirements set forth in paragraph

(a), a person seeking to qualify as a write-in candidate shall,

at the time of subscribing to the oath or affirmation, state in

9

writing:

     1. The party of which the person is a member. If the person

11

is not a member of any party, that person shall so indicate by

12

writing "no party affiliation."

     2. That the person has not been a registered member of any

14

other political party at any time during the 90 days immediately

15

preceding that date.
