Section 1. Health Care Transition Services Task Force for
9
Youth and Young Adults with Disabilities.--
10
(1) The Department of Health shall create a statewide
11
Health Care Transition Services Task Force for Youth and Young
12
Adults with Disabilities. It is the intent of the Legislature
13
that the task force assess the need for health care transition
14
services for youth with disabilities, develop strategies to
15
ensure successful transition from the pediatric to the adult
16
health care system, and identify existing and potential funding
17
sources. The task force shall be composed of 13 members and
18
shall have representatives from at least three geographic areas
19
in the state, representing rural, suburban, and urban areas.
20
(2) The task force shall be composed of the following
21
persons:
22
(a) The director of the Division of Children's Medical
23
Services Network within the Department of Health or his or her
24
designee who shall be the chairperson of the task force.
25
(b) A representative from the children's health care
26
medical community.
27
(c) A representative from the adult health care medical
28
community.
29
(d) The director of the Agency for Persons with
30
Disabilities or his or her designee.
31
(e) Two representatives of associations that advocate for
32
persons who have chronic medical conditions or disabilities,
33
such as the American Diabetes Association, the Sickle Cell
34
Foundation, the Cystic Fibrosis Foundation, United Cerebral
35
Palsy, the Spina Bifida Association, or the Down Syndrome
36
Association.

(f) Two young adults who have chronic health problems or
38
developmental disabilities or a family member of such young
39
adults.
40
(g) The deputy commissioner of the Division of Vocational
41
Rehabilitation within the Department of Education or his or her
42
designee.
43
(h) The Commissioner of Education or his or her designee.
44
(i) The Secretary of Health Care Administration or his or
45
her designee.
46
(j) A person appointed by the President of the Senate.
47
(k) A person appointed by the Speaker of the House of
48
Representatives.

(3) The Department of Health shall provide staff support
50
to the task force. Task force members who are not public
51
officers or employees shall serve without compensation, but are
52
entitled to reimbursement for per diem and travel expenses
53
incurred as provided in s. 112.061, Florida Statutes, in
54
carrying out their duties, which shall be paid by the Department
55
of Health. Members who are public officers or employees shall be
56
reimbursed by the budget entity through which they are
57
compensated.

(4) The task force shall:
59
(a) Convene by August 31, 2008.
60
(b) Obtain input from key stakeholders, community
61
stakeholders, public agencies, the medical practice community,
62
and youth who have chronic special health care needs and
63
disabilities and their families to assess the need for health
64
care transition services and to identify barriers that impede
65
access to comprehensive medical treatment and health care for
66
youth and young adults who have chronic special health care
67
needs and disabilities.

(c) Develop a statewide plan to promote the development of
69
health care transition services. The plan should put forth
70
different models that accommodate the geographic and cultural
71
diversity in the state and that are adapted to the local needs
72
of communities and to local health services delivery systems.
73
Furthermore, the plan should promote the integration of health
74
care transition services with transition programs for education,
75
vocation, and independent living.
76
(d) Identify common or comparable performance measures for
77
all entities that serve the needs for health care transition
78
services of youth and young adults who have chronic special
79
health care needs and disabilities.

(e) Collect and disseminate information concerning best
81
practices in health care transition services for youth and young
82
adults who have chronic special health care needs and
83
disabilities.
84
(f) Identify existing and potential funding sources to
85
create health care transition services within communities.
86
(5) The task force shall present a final report of its
87
findings and recommendations to the Governor, the President of
88
the Senate, and the Speaker of the House of Representatives by
89
January 1, 2009, and shall expire upon the submission of the
90
report.

Section 2. This act shall take effect July 1, 2008.
