       Senator Bennett moved the following:
       
    1         Senate Amendment 
    2  
    3         Delete lines 244 - 251
    4  and insert:
    5         (c) The insurer shall attach a cover page to any annuity
    6  policy informing the purchaser of the unconditional refund
    7  period prescribed in paragraph (b). The cover page must also
    8  provide contact information for the issuing company and the
    9  selling agent, the department’s toll-free help line, and the
   10  disclosure required in s. 627.4554(10). The cover page is part
   11  of the contract and is subject to review by the office pursuant
   12  to s. 627.410.
