| 9 | 2. The Agency for Persons with Disabilities. |
| 10 | 3. The Department of Children and Family Services. |
| 11 | 4. The Department of Corrections. |
| 12 | 5. The Department of Elderly Affairs. |
| 13 | 6. The Department of Health. |
| 14 | 7. The Department of Juvenile Justice. |
| 15 | 8. The Bureau of Pharmacy Services within the Agency for |
| 16 | Health Care Administration, which shall be represented by the |
| 17 | bureau chief. |
| 18 | 9. The Bureau of Statewide Pharmaceutical Services within |

| 26 | 3. A clinical pharmacy program manager from the Agency for |

| 28 | 4. The chair of the Department of Health's Pharmacy and |
| 29 | Therapeutics Committee. |
| 30 | 5. The general counsel for the Agency for Health Care |
| 31 | Administration or his or her designee. |
| 32 | 6. The general counsel for a state agency in the executive |
| 33 | branch of state government, or his or her designee. |
| 34 | 7. A representative from the Executive Office of the |
| 35 | Governor. |
| 36 | 8. The statewide pharmacy director of the Department of |
| 37 | Corrections' Office of Health Services. |
| 38 | 9. Three members appointed by the Speaker of the House of |
| 39 | Representatives, one of whom must be an active licensed Florida |
| 40 | pharmacist who has been employed as a pharmacist for at least |
| 41 | the past 5 years, and one of whom must have experience as a |
| 42 | Florida pharmaceutical repackager, licensed by the United States |
| 43 | Food and Drug Administration, for at least the past 5 years. |
| 44 | 10. Three members appointed by the President of the |
| 45 | Senate, one whom must be an active licensed Florida pharmacist |
| 46 | who has been employed as a pharmacist for at least the past 5 |
| 47 | years, and one of whom must have experience as a Florida |
| 48 | pharmaceutical repackager, licensed by the United States Food |
| 49 | and Drug Administration, for at least the past 5 years. |
