8through (aaaa) are added to that subsection, and subsections
9(13) and (14) are added to that section, to read:
10     320.08056  Specialty license plates.--
11     (4)  The following license plate annual use fees shall be
12collected for the appropriate specialty license plates:
13     (cc)  Choose Life license plate, $25 $20.
14     (qqq)  Autism license plate, $25.
15     (rrr)  Let's Go Surfing license plate, $25.
16     (sss)  Florida Horse Park license plate, $25.
17     (ttt)  St. Johns River license plate, $25.
18     (uuu)  Endless Summer license plate, $25.

20     (www)  Fraternal Order of Police license plate, $25.

26     (13)  Effective July 1, 2011, in addition to the
27requirements of subsections (10) and (11) and notwithstanding
28any specific authority provided in s. 320.08058, an organization
29or entity receiving annual use fee revenue from the sale of a
30specialty license plate may use no more than a total of 10
31percent of the revenues for administrative, marketing, and
32promotion costs, including annual audit and compliance affidavit
33costs.
34     (14)  Notwithstanding any other provision of this chapter,
35no annual use fee revenue derived from the sale of a specialty
36license plate, or the interest thereon, shall be expended on
37projects or programs outside this state.
38     Section 2.  Subsection (29), paragraph (b) of subsection
39(32), and subsection (35) of section 320.08058, Florida
40Statutes, are amended, and subsections (69) through (79) are
41added to that section, to read:
42     320.08058  Specialty license plates.--
43     (29)  CHOOSE LIFE LICENSE PLATES.--
44     (a)  The department shall develop a Choose Life license
45plate as provided in this section. The word "Florida" must
46appear at the bottom of the plate, and the words "Choose Life"
47must appear at the top of the plate.
48     (b)  The annual use fees shall be distributed quarterly to
49Choose Life, Inc., a s. 501(c)(3) nonprofit corporation, with a
50list of annually to each county in the ratio that the annual use
51fees collected by each county bears to the total fees collected
52for the plates within the state.
53     (c)  Up to 10 percent of the fees may be used for
54administrative costs directly associated with the operations of
55Choose Life, Inc., and activities by Choose Life, Inc., to
56promote adoption and to market the license plate in this state.
57Choose Life, Inc., shall administer distribution of the
58remaining Each county shall distribute the funds to
59nongovernmental, not-for-profit agencies within the county,
60which agencies' services primarily consist of are limited to
61counseling and meeting the physical needs of pregnant women who
62are considering committed to placing their children for
63adoption.
64     1.  Funds may not be distributed to any agency that is
65involved or associated with abortion activities, including
66counseling for or referrals to abortion clinics, providing
67medical abortion-related procedures, or proabortion advertising,
68and funds may not be distributed to any agency that charges
69women for services received.
70     2.  Choose Life, Inc., may, from time to time, establish
71criteria pertaining to appropriate percentages of expenditures
72on items including, but not limited to, physical needs of women,
73physical needs of infants awaiting adoption, counseling,
74advertising, and training.
75     3.  Choose Life license plate funds generated in counties
76that do not have a qualified service provider shall be
77distributed by Choose Life, Inc., to qualified service providers
78in other counties.
79     4.  Funds shall not be removed from a county with active
80qualified service providers for distribution to service
81providers in other counties unless year-end unused fund balances
82exceed 20 percent of the county's annual Choose Life license
83plate revenues for 3 consecutive years. If year-end fund
84balances exceed 20 percent of a county's annual revenues for 3
85consecutive years, such balance may be distributed by Choose
86Life, Inc., to qualified service providers in other counties.
87     1.  Agencies that receive the funds must use at least 70
88percent of the funds to provide for the material needs of
89pregnant women who are committed to placing their children for
90adoption, including clothing, housing, medical care, food,
91utilities, and transportation. Such funds may also be expended
92on infants awaiting placement with adoptive parents.
93     2.  The remaining funds may be used for adoption,
94counseling, training, or advertising, but may not be used for
95administrative expenses, legal expenses, or capital
96expenditures.
97     5.3.  Each service provider agency that receives such funds
98must submit an annual attestation to Choose Life, Inc., which
99shall, upon request, make copies available to a state agency
100charged with auditing the county. Any unused funds that exceed
10110 percent of the funds received by an agency during its fiscal
102year must be returned to the county, which shall distribute them
103to other qualified agencies.
104     6.  Choose Life license plate use fees generated in this
105state may only be spent on activities within this state.
106     (d)1.  Within 90 days after the effective date of this
107paragraph, county commissions that have never distributed Choose
108Life funds shall forward such funds to Choose Life, Inc., for
109distribution.
110     2.  Within 180 days after the effective date of this
111paragraph, county commissions that have been distributing Choose
112Life funds shall forward fund balances to Choose Life, Inc., for
113distribution.
114     (32)  UNITED WE STAND LICENSE PLATES.--
115     (b)  The department shall retain all revenues from the sale
116of such plates until all startup costs for developing and
117issuing the plates have been recovered. Thereafter,
118notwithstanding s. 332.14, 100 percent of the annual use fee
119shall be distributed to the Department of Transportation SAFE
120Council for the development and implementation of programs to
121fund a grant program to enhance security at airports throughout
122the state, pursuant to s. 332.14. Up to 10 percent of the annual
123use fee revenue may be used for administration, promotion, and
124marketing of the plate, including annual audit and compliance
125affidavit costs.
126     (35)  FLORIDA GOLF LICENSE PLATES.--
127     (a)  The Department of Highway Safety and Motor Vehicles
128shall develop a Florida Golf license plate as provided in this
129section. The word "Florida" must appear at the bottom of the
130plate. The Dade Amateur Golf Association, following consultation
131with the PGA TOUR, the Florida Sports Foundation, the LPGA, and
132the PGA of America may submit a revised sample plate for
133consideration by the department.
134     (b)  The department shall distribute the Florida Golf
135license plate annual use fee to the Dade Amateur Golf
136Association, a s. 501(c)(3) organization Florida Sports
137Foundation, a direct-support organization of the Office of
138Tourism, Trade, and Economic Development. The license plate
139annual use fees are to be annually allocated as follows:
140     1.  Up to 10 5 percent of the proceeds from the annual use
141fees may be used by the Dade Amateur Golf Association Florida
142Sports Foundation for the administration of the Florida Junior
143Youth Golf Program.
144     2.  The Dade Amateur Golf Association shall receive the
145first $80,000 in proceeds from the annual use fees for the
146operation of youth golf programs in Miami-Dade County.
147Thereafter, 15 percent of the proceeds from the annual use fees
148shall be provided to the Dade Amateur Golf Association for the
149operation of youth golf programs in Miami-Dade County.
150     3.  The remaining proceeds from the annual use fees shall
151be available for grants to nonprofit organizations to operate
152youth golf programs and for marketing the Florida Golf license
153plates. All grant recipients, including the Dade Amateur Golf
154Association, shall be required to provide to the Dade Amateur
155Golf Association Florida Sports Foundation an annual program and
156financial report regarding the use of grant funds. Such reports
157shall be made available to the public.
158     (c)  The Dade Amateur Golf Association shall Florida Sports
159Foundation may establish a Florida Junior Golf Council Youth
160Golf Program. The Florida Junior Golf Council Youth Golf Program
161shall assist organizations for the benefit of youth, introduce
162young people to golf, instruct young people in golf, teach the
163values of golf, and stress life skills, fair play, courtesy, and
164self-discipline.
165     (d)  The Dade Amateur Golf Association Florida Sports
166Foundation shall establish a seven-member Florida Junior Golf
167Council committee to offer advice regarding the distribution of
168the annual use fees for grants to nonprofit organizations. The
169council advisory committee shall consist of one member from a
170group serving youth, one member from a group serving disabled
171youth, and five members at large.
172     (69)  AUTISM LICENSE PLATES.--
173     (a)  The department shall develop an Autism license plate
174as provided in this section. Autism license plates must bear the
175colors and design approved by the department. The word "Florida"
176must appear at the top of the plate, and the words "Support
177Autism Programs" must appear at the bottom of the plate.
178     (b)  The proceeds from the license plate annual use fee
179shall be distributed to Achievement and Rehabilitation Centers,
180Inc., to fund service programs for autism and related
181disabilities throughout the state and to operate and establish
182programs to support individuals with autism and related
183disabilities through direct services, evaluation, training, and
184awareness. Achievement and Rehabilitation Centers, Inc., shall
185establish an Autism Services Grant Council that shall provide
186grants from available Autism license plate proceeds to nonprofit
187organizations for direct services and programs for individuals
188with autism and related disabilities and their families.
189Consideration for participation in such services and programs
190shall be given to applicants who are children or adults with
191autism and related disabilities and their families and shall
192include those who are on the Agency for Persons with
193Disabilities waiting lists for services. Achievement and
194Rehabilitation Centers, Inc., shall also establish an Autism
195License Plate Fund. Moneys in the fund shall be used by the
196grant council as provided in this paragraph. All funds received
197under this subsection must be used in this state.
198     (c)  Achievement and Rehabilitation Centers, Inc., may
199retain all proceeds from the annual use fee up to $85,000 until
200all documented startup costs for developing and establishing the
201plate have been recovered. Thereafter, the proceeds from the
202annual use fee shall be used as follows:
203     1.  Up to 10 percent of the proceeds from the annual use
204fee may be used for the administration of the Autism License
205Plate Fund, the Autism Services Grant Council, and related
206matters.
207     2.  Funds may be used as necessary for annual audit or
208compliance affidavit costs.
209     3.  Thirty-five percent of the proceeds from the annual use
210fee shall be used to establish and operate programs to support
211individuals with autism and related disabilities and their
212families through direct services, evaluation, training, and
213awareness in the state.
214     4.  The Center for Autism and Related Disabilities at the
215University of Miami shall receive 15 percent of the proceeds
216from the annual use fee for distribution, as determined
217appropriate by the director of that center, to the seven
218regional autism centers created under s. 1004.55. The regional
219centers shall use the proceeds to support the services they
220provide.
221     5.  The remaining proceeds from the annual use fee shall be
222available to the Autism Services Grant Council for grants to
223nonprofit organizations to operate direct services programs for
224individuals with autism and related disabilities and their
225families and for marketing the Autism license plate. All grant
226recipients, including Achievement and Rehabilitation Centers,
227Inc., and the Center for Autism and Related Disabilities, shall
228be required to provide to the Autism Services Grant Council an
229annual program and financial report regarding the use of grant
230funds. Such reports shall be made available to the public.
231     (d)  Notwithstanding any provision of law to the contrary,
232the first $10,000 in proceeds generated from voluntary
233contributions to the Autism License Plate Fund authorized in ss.
234320.02 and 322.08 shall be retained by the department to offset
235the department's implementation costs.
236     (70)  LET'S GO SURFING LICENSE PLATES.--
237     (a)  The department shall develop a Let's Go Surfing
238license plate as provided in this section. Let's Go Surfing
239license plates must bear the colors and design approved by the
240department. The word "Florida" must appear at the top of the
241plate, and the words "Let's Go Surfing" must appear at the
242bottom of the plate.
243     (b)  The license plate annual use fees shall be distributed
244to East Coast Surfing Hall of Fame and Museum, Inc., to fund
245activities, programs, and projects to promote Florida's surfing-
246related tourism, preserve Florida's surfing history and
247heritage, preserve Florida's surf and shores, and honor
248lifeguards who patrol Florida's surf and shores to protect
249beachgoers, surfers, and others. East Coast Surfing Hall of Fame
250and Museum, Inc., may retain all revenue from the annual use
251fees until all startup costs for developing and establishing the
252plate have been recovered. Thereafter, up to 10 percent of the
253annual use fee revenue may be used for administration,
254promotion, and marketing of the plate, including annual audit
255and compliance affidavit costs.
256     (71)  FLORIDA HORSE PARK LICENSE PLATES.--
257     (a)  The department shall develop a Florida Horse Park
258license plate as provided in this section. Florida Horse Park
259license plates must bear the colors and design approved by the
260department. The word "Florida" must appear at the top of the
261plate, and the words "Discover Florida's Horses" must appear at
262the bottom of the plate.
263     (b)  The annual use fees shall be distributed to the
264Florida Agriculture Center and Horse Park Authority created by
265s. 570.952, which shall retain all proceeds until all startup
266costs for developing and establishing the plate have been
267recovered. Thereafter, the proceeds shall be used as follows:
268     1.  A maximum of 10 percent of the proceeds from the annual
269use fees may be used for the administration, promotion, and
270marketing of the Florida Horse Park license plate.
271     2.  The remaining proceeds shall be used by the authority
272to promote the Florida Agriculture Center and Horse Park located
273in Marion County; to support continued development of the park,
274including the construction of additional educational facilities,
275barns, and other structures; to provide improvements to the
276existing infrastructure at the park; and to provide for
277operational expenses of the Florida Agriculture Center and Horse
278Park.
279     (72)  ST. JOHNS RIVER LICENSE PLATES.--
280     (a)  The department shall develop a St. Johns River license
281plate as provided in this section. St. Johns River license
282plates must bear the colors and design approved by the
283department. The word "Florida" must appear at the top of the
284plate, and the words "St. Johns River" must appear at the bottom
285of the plate.
286     (b)  The requirements of s. 320.08053 must be met prior to
287the issuance of the plate. Thereafter, the license plate annual
288use fees shall be distributed to the St. Johns River Alliance,
289Inc., a s. 501(c)(3) nonprofit organization, which shall
290administer the fees as follows:
291     1.  The St. Johns River Alliance, Inc., shall retain the
292first $60,000 of the annual use fees as direct reimbursement for
293administrative costs, startup costs, and costs incurred in the
294development and approval process. Thereafter, up to 10 percent
295of the annual use fee revenue may be used for administrative
296costs directly associated with education programs, conservation,
297research, and grant administration of the organization and for
298promotion and marketing of the specialty license plate.
299     2.  At least 30 percent of the fees shall be available for
300competitive grants for targeted community-based or county-based
301research or projects for which state funding is limited or not
302currently available. The remaining 50 percent shall be directed
303toward community outreach and access programs. The competitive
304grants shall be administered and approved by the board of
305directors of the St. Johns River Alliance, Inc. A grant advisory
306committee shall be composed of six members chosen by the St.
307Johns River Alliance board members.
308     3.  Any remaining funds shall be distributed with the
309approval of and accountability to the board of directors of the
310St. Johns River Alliance, Inc., and shall be used to support
311activities contributing to education, outreach, and springs
312conservation.
313     (73)  ENDLESS SUMMER LICENSE PLATES.--
314     (a)  The department shall develop an Endless Summer license
315plate as provided in this section. Endless Summer license plates
316must bear the colors and design approved by the department. The
317word "Florida" must appear at the top of the plate, and the
318words "Endless Summer" must appear at the bottom of the plate.
319     (b)  The license plate annual use fees shall be distributed
320to Surfing's Evolution & Preservation Corporation to fund its
321activities, programs, and projects aimed at preserving the sport
322of surfing. Surfing's Evolution & Preservation Corporation may
323retain all revenue from the annual use fees until all startup
324costs for developing and establishing the plate have been
325recovered. Thereafter, up to 10 percent of the annual use fee
326revenue may be used for promotion and marketing of the specialty
327license plate and administrative costs directly associated with
328the corporation's programs and the specialty license plate.
329Surfing's Evolution & Preservation Corporation shall use the
330remaining funds as follows:
331     1.  To fund the proposed Surfing's Evolution & Preservation
332Experience project.
333     2.  To provide funds for the provision of lifeguards or the
334building of artificial reefs.
335     3.  To provide funds to organizations that house the
336history and artifacts of surfing or promote the sport through
337exhibits, lectures, and events.
338     4.  To support programs and events of other organizations
339that support beaches and oceans and promote education on beach
340safety, coastal pollution, and beach ecology.
341     (74)  CATCH ME, RELEASE ME LICENSE PLATES.--
342     (a)  The department shall develop a Catch Me, Release Me
343license plate as provided in this section. Catch Me, Release Me
344license plates must bear the colors and design approved by the
345department. The word "Florida" must appear at the top of the
346plate, and the words "Catch Me, Release Me" must appear at the
347bottom of the plate.
348     (b)  The license plate annual use fees shall be distributed
349to the Guy Harvey Ocean Foundation, Inc., to fund marine-related
350scientific research, including research of free-ranging pelagic
351marine species that inhabit, use, or migrate through Florida
352waters; conservation initiatives; and education and public
353outreach programs targeting school-aged children in the state.
354The Guy Harvey Ocean Foundation, Inc., may retain all revenue up
355to $60,000 from the annual use fees until all startup costs for
356developing and establishing the plate have been recovered.
357Thereafter, up to 10 percent of the annual use fee revenue may
358be used for administrative costs directly associated with the
359operations of the Guy Harvey Ocean Foundation, Inc., and up to
36010 percent may be used for promotion and marketing of the
361specialty license plate.
362     (75)  FRATERNAL ORDER OF POLICE LICENSE PLATES.--
363     (a)  The department shall develop a Fraternal Order of
364Police license plate as provided in this section. Fraternal
365Order of Police license plates must bear the colors and design
366approved by the department. The word "Florida" must appear at
367the top of the plate, and the words "Fraternal Order of Police"
368must appear at the bottom of the plate.
369     (b)  In order to purchase the Fraternal Order of Police
370license plate, a person must submit a certified letter from the
371Florida State Lodge of the Fraternal Order of Police showing
372that he or she qualifies for the use of the Fraternal Order of
373Police law enforcement emblem by being a member in good
374standing.
375     (c)  The license plate annual use fees shall be distributed
376to the Florida State Lodge Fraternal Order of Police Memorial
377Foundation to fund its activities, programs, and projects aimed
378at preserving, maintaining, and expanding the Florida Fraternal
379Order of Police Law Enforcement Memorial. The Florida State
380Lodge of the Fraternal Order of Police may retain all revenue
381from the annual use fees until all startup costs for developing
382and establishing the plate have been recovered. Thereafter, up
383to 10 percent of the annual use fee revenue may be used for
384promotion and marketing of the specialty license plate and
385administrative costs directly associated with memorial programs
386and the specialty license plate. The Florida State Lodge  
387Fraternal Order of Police Memorial Foundation shall use the
388remaining funds as follows:
389     1.  To fund the Florida State Fraternal Order of Police Law
390Enforcement Memorial projects.
391     2.  To provide funds for the expansion, maintenance, and
392support of the Florida State Lodge Fraternal Order of Police
393Memorial Foundation.
394     3.  To provide funds to organizations that help support the
395families of law enforcement officers who have died in the line
396of duty.
397     4.  To support programs and events that support the Florida
398Fraternal Order of Police Law Enforcement Memorial.
399     (76)  GO GREEN FLORIDA LICENSE PLATES.--
400     (a)  The department shall develop a Go Green Florida
401license plate as provided in this section. Go Green Florida
402license plates must bear the colors and design approved by the
403department. The word "Florida" must appear at the top of the
404plate, the words "Go Green" must appear on the bottom of the
405plate, and the "Go Green Florida" emblem or logo must appear on
406the plate.
407     (b)  The requirements of s. 320.08053 must be met prior to
408the issuance of the plate. The annual use fees shall be
409distributed to the Coalition for Renewable Energy Solutions,
410Inc. After reimbursement for documented costs expended to
411establish the plate, the proceeds of the annual use fees shall
412be used in the following manner:
413     1.  Up to 10 percent may be used by the Coalition for
414Renewable Energy Solutions, Inc., for administrative, promotion,
415and marketing costs.
416     2.  The remainder shall be used to fund the Renewable
417Energy and Energy-Efficient Technologies Grants Program pursuant
418to s. 377.804, the Solar Energy System Incentives Program
419pursuant to s. 377.806, under the Florida Energy and Climate
420Commission, or other educational programs dedicated to providing
421effective alternative energy practices.
422     (c)  The coalition shall comply with the financial audit
423requirements of s. 320.08062.
424     (77)  FLORIDA BIODIVERSITY FOUNDATION LICENSE PLATES.--
425     (a)  The department shall develop a Florida Biodiversity
426Foundation license plate as provided in this section. Florida
427Biodiversity Foundation license plates must bear the colors and
428design approved by the department. The word "Florida" must
429appear at the top of the plate, and the words "Save Wild
430Florida" must appear at the bottom of the plate.
431     (b)  The annual use fees shall be distributed to the
432Florida Biodiversity Foundation, Inc., which shall retain 50
433percent of the proceeds until the startup costs to develop and
434establish the plates have been recovered. Thereafter, the
435proceeds shall be used as follows:
436     1.  A maximum of 10 percent of the proceeds may be used to
437fund the administrative, promotion, and marketing costs of the
438license plate program and the foundation.
439     2.  The remaining fees shall be used by the foundation to
440fund research, education, and scientific study of the diversity
441of animals and plants and to aid in the preservation, study,
442conservation, and recovery of imperiled organisms.
443     (78)  TOOMEY FOUNDATION FOR THE NATURAL SCIENCES LICENSE
444PLATES.--
445     (a)  The department shall develop a Toomey Foundation for
446the Natural Sciences license plate as provided in this section.
447Toomey Foundation for the Natural Sciences license plates must
448bear the colors and design approved by the department. The word
449"Florida" must appear at the top of the plate, and the words
450"Preserving the Past" must appear at the bottom of the plate.
451     (b)  The annual use fees shall be distributed to the Toomey
452Foundation for the Natural Sciences, Inc., which shall retain 50
453percent of the proceeds until the startup costs to develop and
454establish the plates have been recovered. Thereafter, the
455proceeds shall be used as follows:
456     1.  A maximum of 10 percent of the proceeds may be used to
457fund the administrative, promotion, and marketing costs of the
458license plate program and the foundation.
459     2.  The remaining fees shall be used by the foundation to
460acquire, manage, and preserve properties that are of scientific
461importance to paleontologists, archeologists, or geologists for
462the purposes of research and education and to promote and
463protect the unique natural sciences of this state.
464     (79)  TRINITY LICENSE PLATES.--
465     (a)  The department shall develop a Trinity license plate
466as provided in this section. Trinity license plates must bear
467the colors and design approved by the department. The word
468"Florida" must appear at the top of the plate, and the words
469"Sunshine State" must appear at the bottom of the plate.
470     (b)  The annual use fees shall be distributed to the Toomey
471Foundation for the Natural Sciences, Inc., which shall retain 50
472percent of the proceeds until the startup costs to develop and
473establish the plates have been recovered. Thereafter, the
474proceeds shall be used as follows:
475     1.  A maximum of 10 percent of the proceeds may be used to
476fund the administrative, promotion, and marketing costs of the
477license plate program and the foundation.
478     2.  The remaining fees shall be used by the foundation to
479support educational, research, and scientific activities in this
480state, including the distribution of funds to qualified entities
481in furtherance of the stated purposes of the foundation.
482     Section 3.  Paragraphs (h) and (i) are added to subsection
483(15) of section 320.02, Florida Statutes, to read:
484     320.02  Registration required; application for
485registration; forms.--
486     (15)
487     (h)  The application form for motor vehicle registration
488and renewal of registration must include language permitting a
489voluntary contribution of $5 to the Autism License Plate Fund
490administered by Achievement and Rehabilitation Centers, Inc.
491Except as provided in s. 320.08058(69)(d), such contributions
492must be distributed by the department each month to the Autism
493License Plate Fund administered by Achievement and
494Rehabilitation Centers, Inc., to be used as provided in s.
495320.08058(69).
496     (i)  The application form for motor vehicle registration
497and renewal of registration must include language permitting a
498voluntary contribution of $1 to Ronald McDonald Houses of
499Florida. Such contributions must be distributed by the
500department each month to Ronald McDonald House Charities Tampa
501Bay, Inc., a not-for-profit organization.

503For the purpose of applying the service charge provided in s.
504215.20, contributions received under this subsection are not
505income of a revenue nature.
506     Section 4.  Section 320.0706, Florida Statutes, is amended
507to read:
508     320.0706  Display of license plates on trucks.--The owner
509of any commercial truck of gross vehicle weight of 26,001 pounds
510or more shall display the registration license plate on both the
511front and rear of the truck in conformance with all the
512requirements of s. 316.605 that do not conflict with this
513section. The owner of a dump truck may place the rear license
514plate on the gate no higher than 60 inches to allow for better
515visibility. However, the owner of a truck tractor shall be
516required to display the registration license plate only on the
517front of such vehicle. The owner of any wrecker issued a regular
518wrecker license plate shall display the license plate only on
519the front of the vehicle. The owner of any wrecker issued gross
520vehicle weight wrecker license plates shall display the license
521plates on both the front and rear of the vehicle in conformance
522with the requirements of s. 316.605 that do not conflict with
523this section. A violation of this section is a noncriminal
524traffic infraction, punishable as a moving violation as provided
525in chapter 318.
526     Section 5.  Subsection (6) of section 322.08, Florida
527Statutes, is amended to read:
528     322.08  Application for license.--
529     (6)  The application form for a driver's license or
530duplicate thereof shall include language permitting the
531following:
532     (a)  A voluntary contribution of $1 per applicant, which
533contribution shall be deposited into the Health Care Trust Fund
534for organ and tissue donor education and for maintaining the
535organ and tissue donor registry.
536     (b)  A voluntary contribution of $1 per applicant, which
537contribution shall be distributed to the Florida Council of the
538Blind.
539     (c)  A voluntary contribution of $2 per applicant, which
540shall be distributed to the Hearing Research Institute,
541Incorporated.
542     (d)  A voluntary contribution of $1 per applicant, which
543shall be distributed to the Juvenile Diabetes Foundation
544International.
545     (e)  A voluntary contribution of $1 per applicant, which
546shall be distributed to the Children's Hearing Help Fund.
547     (f)  A voluntary contribution of $1 per applicant, which
548shall be distributed to Family First, a nonprofit organization.
549     (g)  A voluntary contribution of $5 per applicant, which,
550except as provided in s. 320.08058(69)(d), shall be distributed
551to the Autism License Plate Fund administered by Achievement and
552Rehabilitation Centers, Inc., to be used as provided in s.
553320.08058(69).

555A statement providing an explanation of the purpose of the trust
556funds shall also be included. For the purpose of applying the
557service charge provided in s. 215.20, contributions received
558under paragraphs (b), (c), (d), (e), and (f), and (g) and under
559s. 322.18(9)(a) are not income of a revenue nature.
560     Section 6.  This act shall take effect September 1, 2009.
