The Committee on Ethics and Elections (Joyner) recommended the
following:
1 Senate Amendment (with title amendment)

3 Delete lines 14 - 37
4 and insert:

6 (1) A voter information card shall be furnished by the
7 supervisor to all registered voters residing in the supervisor’s
8 county. The card must contain:
9 (a) Voter’s registration number.
10 (b) Date of registration.
11 (c) Full name.
12 (d) Party affiliation.
13 (e) Date of birth.
14 (f) Address of legal residence.
15 (g) Precinct number.
16 (h) Polling place address.
17 (i)(h) Name of supervisor and contact information of
18 supervisor.
19 (j)(i) Other information deemed necessary by the
20 supervisor.
21 (2) A voter may receive a replacement voter information
22 card by providing a signed, written request for a replacement
23 card to a voter registration official. Upon verification of
24 registration, the supervisor shall issue the voter a duplicate
25 card without charge.
26 (3) In the case of a change of name, address of legal
27 residence, polling place address, or party affiliation, the
28 supervisor shall issue the voter a new voter information card.

30 And the title is amended as follows:

34 s. 97.071, F.S.; requiring voter information cards to
35 contain the address of the polling place of the registered
36 voter; requiring a supervisor of elections to issue a new voter
37 information card to a voter upon a change in a voter's address
38 of legal residence or a change in a voter's polling place
39 address; providing an effective date.
