    1                        A bill to be entitled                      
    2         An act relating to voter information cards; amending
    3         s. 97.071, F.S.; requiring the Department of State to
    4         prescribe by rule a uniform statewide voter
    5         information card; requiring voter information cards to
    6         contain the address of the polling place of the
    7         registered voter; providing an effective date.
    8         
    9  Be It Enacted by the Legislature of the State of Florida:
   10         
   11         Section 1. Section 97.071, Florida Statutes, is amended to
   12  read:
   13         97.071 Voter information card.—
   14         (1) The department shall prescribe by rule a uniform
   15  statewide voter information card. The A voter information card
   16  shall be furnished by the supervisor to all registered voters
   17  residing in the supervisor's county. The card must contain:
   18         (a) Voter's registration number.
   19         (b) Date of registration.
   20         (c) Full name.
   21         (d) Party affiliation.
   22         (e) Date of birth.
   23         (f) Address of legal residence.
   24         (g) Precinct number.
   25         (h)Polling place address.
   26         (i)(h) Name of supervisor and contact information of
   27  supervisor.
   28         (j)(i) Other information deemed necessary by the department
   29  supervisor.
   30         (2) A voter may receive a replacement voter information
   31  card by providing a signed, written request for a replacement
   32  card to a voter registration official. Upon verification of
   33  registration, the supervisor shall issue the voter a duplicate
   34  card without charge.
   35         (3) In the case of a change of name, address, or party
   36  affiliation, the supervisor shall issue the voter a new voter
   37  information card.
   38         Section 2. This act shall take effect July 1, 2009.
