9 | requiring the application form for a driver's license or |
10 | duplicate thereof to include language permitting the |
11 | applicant to make a voluntary contribution to Senior |
12 | Vision Services, for services for persons with |
13 | developmental disabilities, and to the Ronald McDonald |
14 | House; providing for distribution of funds collected from |
15 | voluntary contributions; providing that such contributions |
16 | are not considered income of a revenue nature; providing |
17 | an effective date. |

19 | Be It Enacted by the Legislature of the State of Florida: |
20 |

21 | Section 1. Paragraphs (i), (j), and (k) are added to |
22 | subsection (15) of section 320.02, Florida Statutes, to read: |
23 | 320.02 Registration required; application for |
24 | registration; forms.- |
25 | (15) |
26 | (i) The application form for motor vehicle registration |
27 | and renewal of registration must include language permitting a |
28 | voluntary contribution of $1 to Blind Babies and Blind Youth |
29 | Services. Such contributions shall be transferred by the |
30 | department each month to the Florida Association of Agencies |
31 | Serving the Blind, Inc., a not-for-profit organization. |
32 | (j) The application form for motor vehicle registration |
33 | and renewal of registration must include language permitting a |
34 | voluntary contribution of $1 for services for persons with |
35 | developmental disabilities. Such contributions shall be |
36 | transferred by the department to The Arc of Florida to be used |
37 | by that organization for programs and services in this state for |
38 | persons with developmental disabilities. |
39 | (k) The application form for motor vehicle registration |
40 | and renewal of registration must include language permitting a |
41 | voluntary contribution of $1 to the Ronald McDonald House. Such |
42 | contributions shall be transferred by the department each month |
43 | to Ronald McDonald House Charities of Tampa Bay, Inc. |

45 | For the purpose of applying the service charge provided in s. |
46 | 215.20, contributions received under this subsection are not |
47 | income of a revenue nature. |
48 | Section 2. Subsection (7) of section 322.08, Florida |
49 | Statutes, is amended to read: |
50 | 322.08 Application for license.- |
51 | (7) The application form for a driver's license or |
52 | duplicate thereof shall include language permitting the |
53 | following: |
54 | (a) A voluntary contribution of $1 per applicant, which |
55 | contribution shall be deposited into the Health Care Trust Fund |
56 | for organ and tissue donor education and for maintaining the |
57 | organ and tissue donor registry. |
58 | (b) A voluntary contribution of $1 per applicant, which |
59 | contribution shall be distributed to the Florida Council of the |
60 | Blind. |
61 | (c) A voluntary contribution of $2 per applicant, which |
62 | shall be distributed to the Hearing Research Institute, |
63 | Incorporated. |
64 | (d) A voluntary contribution of $1 per applicant, which |
65 | shall be distributed to the Juvenile Diabetes Foundation |
66 | International. |
67 | (e) A voluntary contribution of $1 per applicant, which |
68 | shall be distributed to the Children's Hearing Help Fund. |
69 | (f) A voluntary contribution of $1 per applicant, which |
70 | shall be distributed to Family First, a nonprofit organization. |
71 | (g) A voluntary contribution of $1 per applicant, to Stop |
72 | Heart Disease, which shall be distributed to the Florida Heart |
73 | Research Institute, a nonprofit organization. |
74 | (h) A voluntary contribution of $1 per applicant to Senior |
75 | Vision Services, which shall be distributed to the Florida |
76 | Association of Agencies Serving the Blind, Inc., a not-for- |
77 | profit organization. |
78 | (i) A voluntary contribution of $1 per applicant for |
79 | services for persons with developmental disabilities, which |
80 | shall be distributed to The Arc of Florida. |
81 | (j) A voluntary contribution of $1 to the Ronald McDonald |
82 | House, which shall be distributed each month to Ronald McDonald |
83 | House Charities of Tampa Bay, Inc. |

85 | A statement providing an explanation of the purpose of the trust |
86 | funds shall also be included. For the purpose of applying the |
87 | service charge provided in s. 215.20, contributions received |
88 | under paragraphs (b)-(j) (b), (c), (d), (e), (f), and (g) and |
89 | under s. 322.18(9) are not income of a revenue nature. |
90 | Section 3. This act shall take effect October 1, 2010. |
