13 | contribution shall be distributed to the Florida Council of the |

15 | (c) A voluntary contribution of $2 per applicant, which |
16 | shall be distributed to the Hearing Research Institute, |
17 | Incorporated. |
18 | (d) A voluntary contribution of $1 per applicant, which |
19 | shall be distributed to the Juvenile Diabetes Foundation |
20 | International. |
21 | (e) A voluntary contribution of $1 per applicant, which |
22 | shall be distributed to the Children's Hearing Help Fund. |
23 | (f) A voluntary contribution of $1 per applicant, which |
24 | shall be distributed to Family First, a nonprofit organization. |
25 | (g) A voluntary contribution of $1 per applicant to Stop |
26 | Heart Disease, which shall be distributed to the Florida Heart |
27 | Research Institute, a nonprofit organization. |
28 | (h) A voluntary contribution of $1 per applicant to Senior |
29 | Vision Services, which shall be distributed to the Florida |
30 | Association of Agencies Serving the Blind, Inc., a not-for- |
31 | profit organization. |
32 | (i) A voluntary contribution of $1 per applicant for |
33 | services for persons with developmental disabilities, which |
34 | shall be distributed to The Arc of Florida. |
35 | (j) A voluntary contribution of $1 to the Ronald McDonald |
36 | House, which shall be distributed each month to Ronald McDonald |
37 | House Charities of Tampa Bay, Inc. |
38 | (k) Notwithstanding s. 322.081, a voluntary contribution |
39 | of $1 per applicant, which shall be distributed to the League |
40 | Against Cancer/La Liga Contra el Cancer, a not-for-profit |
41 | organization. |
42 | (l) A voluntary contribution of $1 per applicant to |
43 | Prevent Child Sexual Abuse, which shall be distributed to |
44 | Lauren's Kids, Inc., a nonprofit organization. |
45 | (m) A voluntary contribution of $1 per applicant, which |
46 | shall be distributed to Prevent Blindness Florida, a not-for- |
47 | profit organization, to prevent blindness and preserve the sight |
48 | of the residents of this state. |
49 | (n) Notwithstanding s. 322.081, a voluntary contribution |
50 | of $1 per applicant to the state homes for veterans, to be |
51 | distributed on a quarterly basis by the department to the State |
52 | Homes for Veterans Trust Fund, which is administered by the |
53 | Department of Veterans' Affairs. |
54 | (o) A voluntary contribution of $1 per applicant for |
55 | Autism Services and Supports. Such contributions must be |
56 | transferred by the department each month to the Achievement and |
57 | Rehabilitation Centers, Inc., Autism Services Fund. |
58 | (p) Notwithstanding s. 26 of chapter 2010-223, Laws of |
59 | Florida, a voluntary contribution of $1 or more per applicant to |
60 | the Auto Club South Traffic Safety Foundation, a nonprofit |
61 | organization. Funds received by the foundation shall be used to |
62 | improve traffic safety culture in communities through effective |
63 | outreach, education, and activities that will save lives, reduce |
64 | injuries, and prevent crashes. The foundation must comply with |
65 | s. 322.081. |
66 | (q) A voluntary contribution of $1 for Support Our Troops. |
67 | The proceeds shall be transferred by the department each month |
68 | to Support Our Troops, Inc. |
69 | (r) A voluntary contribution of $1 per applicant to the |
70 | Disabled American Veterans, Department of Florida, which shall |
71 | be distributed quarterly to Disabled American Veterans, |
72 | Department of Florida, a nonprofit organization. |

74 | A statement providing an explanation of the purpose of the trust |
75 | funds shall also be included. For the purpose of applying the |
76 | service charge provided in s. 215.20, contributions received |
77 | under paragraphs (b)-(r) (b)-(n) are not income of a revenue |
78 | nature. |
