    5         (f) Evidence that a qualified plan has been in existence
    6  for at least 6 years before the effective date of this act and
    7  has had a continuous contract with the agency and the Department
    8  of Elderly Affairs for the Long-Term Care Community Diversion
    9  Pilot Project.
