    3         Delete line 3963
    4  and insert:
    5  409.972 and 409.978, and must encourage plans to use the most
    6  cost-effective modalities for the treatment of chronic disease,
    7  such as peritoneal dialysis over hemodialysis if the patient and
    8  physician choose this form of treatment. Payment rates for
