       The Committee on Budget (Bennett) recommended the following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete lines 536 - 537
    4  and insert:
    5  nursing services. Medical benefits do not include acupuncture as
    6  defined in s. 457.102. Medical benefits do not include massage
    7  as defined in s. 480.033 unless prescribed by a physician and
    8  the prescription includes the diagnosis, the frequency of
    9  treatment, the modalities of treatment, and a statement of
   10  medical necessity for the massage and any modalities. Massage
   11  therapy is covered only if it is a component of a medically
   12  necessary treatment plan, and is not covered in the absence of
   13  treatment or other therapeutic modalities performed by the
   14  prescribing physician.

   24         insurer
