The Committee on Budget (Bennett) recommended the following:
1 Senate Amendment (with title amendment)

3 Delete lines 929 - 935
4 and insert:
5 effort to do so, as documented in the insurer’s file; and
6 f. For medical services or treatment billed by a physician
7 and not provided in a hospital unless such services are rendered
8 by the physician or are incident to his or her professional
9 services and are included on the physician’s bill, including
10 documentation verifying that the physician is responsible for
11 the medical services that were rendered and billed; and.
12 g. For massage, as defined in s. 480.033, which exceeds
13 more than four modalities rendered during one treatment date,
14 more than eight treatment dates during 1 month, or more than 32
15 treatment dates within 4 months of the first massage service and
16 which is unsupported by medical records documenting the medical
17 necessity for each treatment or modality.

23 the time of issuance or renewal; providing a
24 limitation on the number of reimbursable massage
25 treatments; deleting obsolete
