       The Committee on Budget (Negron) recommended the following:
       
    1         Senate Amendment to Amendment (617530) (with title
    2  amendment)

    5  insert:
    6         Section 20. For members of the Florida Senate and the
    7  Florida House of Representatives who are compensated under s.
    8  11.13(1), Florida Statutes:
    9         (1) The member’s share of the state health insurance
   10  premium for the state group health insurance standard plan and
   11  the state group health maintenance organization standard plan
   12  during the coverage period from July 1, 2012, through June 30,
   13  2013, shall be $50 per month for individual coverage and $180
   14  per month for family coverage. For the coverage period January
   15  1, 2013, through June 30, 2013, a member who has family coverage
   16  may choose to retain the benefits described in the current State
   17  Employees’ PPO Plan Group Health Insurance Plan Booklet and
   18  Benefit Document or the current health maintenance organization
   19  contracts and benefit documents, if the member, effective
   20  December 1, 2012, agrees to pay $263.34 per month for family
   21  coverage in lieu of $180 per month for family coverage.
   22         (2) The member’s share of the state health insurance
   23  premium for the state group health insurance high deductible
   24  plan and the state group health maintenance organization high
   25  deductible plan during the coverage period from July 1, 2012,
   26  through June 30, 2013, shall be $15 per month for individual
   27  coverage and $64.30 per month for family coverage.
   28         (3) The member’s share of the state health insurance
   29  premium for the state group health insurance standard plan and
   30  the state group health maintenance organization standard plan
   31  during the coverage period from July 1, 2012, through December
   32  31, 2012, shall be $15 per month for each member participating
   33  in the Spouse Program in accordance with rule 60P-2.0036,
   34  Florida Administrative Code. For the coverage period January 1,
   35  2013, through June 30, 2013, the member’s share of the health
   36  insurance premiums for the standard plans shall, effective
   37  December 1, 2012, be $50 per month for members participating in
   38  the Spouse Program in accordance with rule 60P-2.0036, Florida
   39  Administrative Code. However, for the coverage period January 1,
   40  2013, through June 30, 2013, a member participating in the
   41  Spouse Program may choose to retain the benefits described in
   42  the current State Employees’ PPO Plan Group Health Insurance
   43  Plan Booklet and Benefit Document or the current health
   44  maintenance organization contracts and benefit documents, if the
   45  member, effective December 1, 2012, agrees to pay $81.67 per
   46  month for participation in the Spouse Program in lieu of paying
   47  $50 per month.
   48         (4) The member’s share of the state health insurance
   49  premium for the state group health insurance high deductible
   50  plan and the state group health maintenance organization high
   51  deductible plan during the coverage period from July 1, 2012,
   52  through June 30, 2013, shall be $15 per month for each member
   53  participating in the Spouse Program in accordance with rule 60P
   54  2.0036, Florida Administrative Code.

   58         Delete line 1024
   59  and insert:
   60         provisions and cross-references; specifying the
   61         premium share of state legislators for various state
   62         group health insurance plans; providing an effective
   63         date.
