House Comm: RCS . 02/09/2012 . . . . ————————————————————————————————————————————————————————————————— ————————————————————————————————————————————————————————————————— The Committee on Health Regulation (Fasano) recommended the following: 1 Senate Amendment (with title amendment) 2 3 Delete everything after the enacting clause 4 and insert: 5 Section 1. Legislative intent; health care transition 6 programs and services for adolescents and young adults who have 7 special health care needs.— 8 (1) It is the intent of the Legislature to provide a 9 strategic and comprehensive approach to the development and 10 implementation of effective health care transition programs and 11 services for adolescents and young adults who have special 12 health care needs. The Health Care Transition Services Task 13 Force for Youth and Young Adults with Disabilities developed a 14 statewide plan to promote the development of such health care 15 transition services, in accordance with chapter 2008-211, Laws 16 of Florida, and outlined the plan in its subsequent report. 17 (2) A program called the Florida Health And Transition 18 Services (FloridaHATS) is established within the Division of 19 Children’s Medical Services Network to oversee transitional 20 services in this state using existing state plans in order to 21 implement effective transition services for adolescents and 22 young adults who have special health care needs. As used in this 23 section, the term “adolescents and young adults who have special 24 health care needs” means individuals who are 12 through 26 years 25 of age; who have chronic physical, developmental, behavioral, or 26 emotional conditions; and who require health care or related 27 services of a type or amount beyond that which is generally 28 required by adolescents or young adults. FloridaHATS shall be 29 responsible for: 30 (a) Developing and overseeing a planning and implementation 31 process and the necessary guides for use by community providers 32 and organizations in developing coordinated systems of services 33 for adolescents and young adults who have special health care 34 needs. 35 (b) Developing and disseminating resource guides that 36 outline the various public and private health care financing 37 options, including commercial insurance, and the respective 38 health care benefits. 39 (c) Coordinating with educational institutions, including 40 medical centers, to identify and make available for health care 41 providers existing training programs regarding the principles, 42 objectives, and methods for the successful transition of 43 adolescents and young adults who have special health care needs 44 to adult health care providers. 45 (d) Maintaining and updating the FloridaHATS’s website with 46 web-based materials related to health care transition services 47 which can be used by adolescents and young adults who have 48 special health care needs and their families, providers, and 49 others involved in health care transition. 50 (e) Collecting and disseminating information in specific 51 clinical areas concerning evidence-based practices and best 52 practices for providing health care transition services for 53 adolescents and young adults who have special health care needs. 54 (f) Providing technical assistance to communities, 55 providers, and organizations that are involved in the 56 development and implementation of systems of services for 57 adolescents and young adults who have special health care needs. 58 (g) Developing and disseminating quality improvement and 59 evaluation components to other health care providers. The 60 quality improvement and evaluation components must include a 61 common or comparable set of performance measures for all 62 entities that provide health care transition services for 63 adolescents and young adults who have special health care needs. 64 (h) Establishing a network of experts in the fields of 65 pediatric and adolescent medicine, adult medicine, and allied 66 health to provide technical assistance and recommendations 67 regarding best practices and policy guidance in health care 68 transition. 69 Section 2. This act shall take effect July 1, 2012. 70 71 ================= T I T L E A M E N D M E N T ================ 72 And the title is amended as follows: 73 Delete everything before the enacting clause 74 and insert: 75 A bill to be entitled 76 An act relating to health care transition programs and 77 services for adolescents and young adults who have 78 special health care needs; providing legislative 79 intent; establishing a program within the Division of 80 Children’s Medical Services Network in the Department 81 of Health to oversee transitional services in this 82 state using existing state plans in order to implement 83 health care transition programs for adolescents and 84 young adults who have special health care needs; 85 specifying responsibilities of the program with 86 respect to the oversight, implementation, and 87 coordination of the program; providing an effective 88 date.
