Paragraph (r) of subsection (7) of section 7 322.08, Florida Statutes, is created to read: 8 322.08 Application for license; requirements for license 9 and identification card forms.— 10 (7) The application form for an original, renewal, or 11 replacement driver license or identification card shall include 12 language permitting the following: 13 (a) A voluntary contribution of $1 per applicant, which 14 contribution shall be deposited into the Health Care Trust Fund 15 for organ and tissue donor education and for maintaining the 16 organ and tissue donor registry. 17 (b) A voluntary contribution of $1 per applicant, which 18 contribution shall be distributed to the Florida Council of the 19 Blind. 20 (c) A voluntary contribution of $2 per applicant, which 21 shall be distributed to the Hearing Research Institute, 22 Incorporated. 23 (d) A voluntary contribution of $1 per applicant, which 24 shall be distributed to the Juvenile Diabetes Foundation 25 International. 26 (e) A voluntary contribution of $1 per applicant, which 27 shall be distributed to the Children’s Hearing Help Fund. 28 (f) A voluntary contribution of $1 per applicant, which 29 shall be distributed to Family First, a nonprofit organization. 30 (g) A voluntary contribution of $1 per applicant to Stop 31 Heart Disease, which shall be distributed to the Florida Heart 32 Research Institute, a nonprofit organization. 33 (h) A voluntary contribution of $1 per applicant to Senior 34 Vision Services, which shall be distributed to the Florida 35 Association of Agencies Serving the Blind, Inc., a not-for 36 profit organization. 37 (i) A voluntary contribution of $1 per applicant for 38 services for persons with developmental disabilities, which 39 shall be distributed to The Arc of Florida. 40 (j) A voluntary contribution of $1 to the Ronald McDonald 41 House, which shall be distributed each month to Ronald McDonald 42 House Charities of Tampa Bay, Inc. 43 (k) Notwithstanding s. 322.081, a voluntary contribution of 44 $1 per applicant, which shall be distributed to the League 45 Against Cancer/La Liga Contra el Cancer, a not-for-profit 46 organization. 47 (l) A voluntary contribution of $1 per applicant to Prevent 48 Child Sexual Abuse, which shall be distributed to Lauren’s Kids, 49 Inc., a nonprofit organization. 50 (m) A voluntary contribution of $1 per applicant, which 51 shall be distributed to Prevent Blindness Florida, a not-for 52 profit organization, to prevent blindness and preserve the sight 53 of the residents of this state. 54 (n) Notwithstanding s. 322.081, a voluntary contribution of 55 $1 per applicant to the state homes for veterans, to be 56 distributed on a quarterly basis by the department to the State 57 Homes for Veterans Trust Fund, which is administered by the 58 Department of Veterans’ Affairs. 59 (o) A voluntary contribution of $1 per applicant to the 60 Disabled American Veterans, Department of Florida, which shall 61 be distributed quarterly to Disabled American Veterans, 62 Department of Florida, a nonprofit organization. 63 (p) A voluntary contribution of $1 per applicant for Autism 64 Services and Supports, which shall be distributed to Achievement 65 and Rehabilitation Centers, Inc., Autism Services Fund. 66 (q) A voluntary contribution of $1 per applicant to Support 67 Our Troops, which shall be distributed to Support Our Troops, 68 Inc., a Florida not-for-profit organization. 69 (r) A voluntary contribution of $1 or more per applicant to 70 the Auto Club Group Traffic Safety Foundation, Inc., a nonprofit 71 organization. Funds received by the foundation must be used to 72 improve traffic safety culture in communities through effective 73 outreach, education, and activities in the state that will save 74 lives, reduce injuries, and prevent crashes. The foundation must 75 comply with s. 322.081. 76 77 A statement providing an explanation of the purpose of the trust 78 funds shall also be included. For the purpose of applying the 79 service charge provided in s. 215.20, contributions received 80 under paragraphs (b)-(r)(b)-(q)are not income of a revenue 81 nature. 82 83 84 ================= T I T L E A M E N D M E N T ================ 85 And the title is amended as follows: 86 Delete line 128 87 and insert: 88 Periods; providing fees; 322.08, F.S.,; requiring the 89 application forms for motor vehicle registration and 90 renewal of registration and for an original, renewal, 91 or replacement driver license or identification card 92 to include language permitting the applicant to make a 93 voluntary contribution to Auto Club Group Traffic 94 Safety Foundation, Inc., providing that such 95 contributions are not income for specified purposes; 96 providing for use of funds; providing that the 97 foundation must comply with specified provisions; 98 amending s. 320.03, F.S.;
