       The Committee on Appropriations (Hays) recommended the
       following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete lines 609 - 615
    4  and insert:
    5         (6) An applicant who is issued a temporary certificate to
    6  practice as a dentist pursuant to this section must practice
    7  under the indirect supervision, as defined in s. 466.003, of a
    8  dentist licensed pursuant to chapter 466.
    9         (7) The department shall establish by rule application and
   10  renewal fees not to exceed $50 for a temporary certificate
   11  issued under this section.
   12         (8) Application must be made on a form prepared and
   13  furnished by the department.
   14         (9) The department shall adopt rules necessary to implement
   15  this section.

   20  and insert:
   21         to receive a temporary certificate; requiring certain
   22         temporary certificateholders to practice under the
   23         indirect supervision of a dentist; requiring the
