    1         Senate Amendment to Amendment (151688) (with directory and

    4         Between lines 206 and 207
    5  insert:
    6         (10) In addition to the comprehensive package of health
    7  insurance and other benefits required or authorized to be
    8  included in the state group insurance program, the program must
    9  provide coverage for medically necessary prescription and
   10  nonprescription enteral formulas and amino-acid-based elemental
   11  formulas for home use, regardless of the method of delivery or
   12  intake, which are ordered or prescribed by a physician. As used
   13  in this subsection, the term “medically necessary” means the
   14  formula to be covered represents the only medically appropriate
   15  source of nutrition for a patient. Such coverage may not exceed
   16  an amount of $20,000 annually for any insured individual.
