Senator Gibson moved the following:
1 Senate Amendment (with title amendment)

3 Delete lines 31 - 91
4 and insert:
5 (3) In addition to the grants awarded under subsections (1)
6 and (2), up to 20 percent of the funding for the Reducing Racial
7 and Ethnic Health Disparities: Closing the Gap grant program
8 shall be dedicated to projects that address improving racial and
9 ethnic health status within specific Front Porch Florida
10 Communities.
11 (3)(4) Nothing in ss. 381.7351-381.7356 shall prevent a
12 person, entity, or organization within a county or group of
13 counties from separately contracting for the provision of racial
14 and ethnic health promotion, health awareness, and disease
15 prevention services.
16 Section 2. Subsection (2) of section 381.7355, Florida
17 Statutes, is amended to read:
18 381.7355 Project requirements; review criteria.—
19 (2) A proposal must include each of the following elements:
20 (a) The purpose and objectives of the proposal, including
21 identification of the particular racial or ethnic disparity the
22 project will address. The proposal must address one or more of
23 the following priority areas:
24 1. Decreasing racial and ethnic disparities in maternal and
25 infant mortality rates.
26 2. Decreasing racial and ethnic disparities in morbidity
27 and mortality rates relating to cancer.
28 3. Decreasing racial and ethnic disparities in morbidity
29 and mortality rates relating to HIV/AIDS.
30 4. Decreasing racial and ethnic disparities in morbidity
31 and mortality rates relating to cardiovascular disease.
32 5. Decreasing racial and ethnic disparities in morbidity
33 and mortality rates relating to diabetes.
34 6. Increasing adult and child immunization rates in certain
35 racial and ethnic populations.
36 7. Decreasing racial and ethnic disparities in oral health
37 care.
38 8. Decreasing racial and ethnic disparities in morbidity
39 and mortality rates relating to sickle cell disease.
40 9. Decreasing racial and ethnic disparities in morbidity
41 and mortality rates relating to Lupus.
42 10. Decreasing racial and ethnic disparities in morbidity
43 and mortality rates relating to Alzheimer’s disease and
44 dementia.
45 11.10. Improving Improve neighborhood social determinants
46 of health, such as transportation, safety, and food access, as
47 outlined by the Centers for Disease Control and Prevention’s
48 “Tools for Putting Social Determinants of Health into Action.”
49 (b) Identification and relevance of the target population.
50 (c) Methods for obtaining baseline health status data and
51 assessment of community health needs.
52 (d) Mechanisms for mobilizing community resources and
53 gaining local commitment.
54 (e) Development and implementation of health promotion and
55 disease prevention interventions.
56 (f) Mechanisms and strategies for evaluating the project’s
57 objectives, procedures, and outcomes.
58 (g) A proposed work plan, including a timeline for
59 implementing the project.
60 (h) Likelihood that project activities will occur and
61 continue in the absence of funding.
62 Section 3. Present subsections (4) and (5) of section
63 381.7356, Florida Statutes, are redesignated as subsections (5)
64 and (6), respectively, new subsections (4) and (5) are added to
65 that section, and subsection (1) of that section is amended, to
66 read:
67 381.7356 Local matching funds; grant awards.—
68 (1) One or more Closing the Gap grants may be awarded in a
69 county, or in a group of adjoining counties from which a
70 multicounty application is submitted. Front Porch Florida
71 Communities grants may also be awarded in a county or group of
72 adjoining counties that are also receiving a grant award.
73 (4) The department may not establish a minimum amount or a
74 maximum amount for grants and shall determine the amount of each
75 award based on the merits of the application. The department
76 shall ensure that grants are awarded to applicants in various
77 regions of the state.
78 (5) In addition to the requirements provided in s.
79 381.7353, and subject to the availability of state and federal
80 funds in the HIV/AIDS program, the department shall promote
81 synergistic initiatives between the Closing the Gap grant
82 program and the HIV/AIDS program to leverage the expertise of
83 the Closing the Gap grant program. These initiatives may include
84 the establishment of a supplemental grant program whereby
85 persons, entities, or organizations eligible for a Closing the
86 Gap grant under s. 381.7354 may submit to the department a grant
87 proposal, pursuant to the application process established under
88 s. 381.7355, for funding to further the purposes listed in s.
89 381.7355(2)(a) and to promote innovative prevention, treatment,
90 and awareness initiatives for minority populations in
91 metropolitan areas which have a higher prevalence of HIV/AIDS
92 for the purposes of reducing the incidence of the HIV infection
93 in such communities and prioritizing the identification of
94 individuals, in a manner consistent with the clinical guidelines
95 of the federal Health Resources and Services Administration, who
96 are not yet aware of their HIV status.

103 related to Front Porch Florida Communities; amending
104 s. 381.7355, F.S.; adding a priority area that may be
105 addressed in a Closing the Gap grant proposal;
106 amending s. 381.7356, F.S.; removing provisions
107 related to Front Porch Florida Communities;
108 prohibiting the Department of Health from establishing
109 a minimum amount or a maximum amount for Closing the
110 Gap grants; requiring the department to base the
111 amount of each award on the merits of each application
112 and to award grants to applicants in various regions
113 of the state; authorizing the Closing the Gap program
114 and the HIV/AIDS program to promote synergistic
115 initiatives that reduce HIV incidence in minority
116 populations in certain metropolitan areas; providing
117 an effective date.
