Senator Stargel moved the following:
1 Senate Amendment (with title amendment)

3 Delete lines 58 - 79
4 and insert:
5 2. The parent or guardian of a child may refuse to have the
6 child included in the immunization registry by signing an opt
7 out a form obtained from the department, or from the health care
8 practitioner or entity that provides the immunization, which
9 indicates that the parent or guardian does not wish to have the
10 child included in the immunization registry. The health care
11 practitioner or entity must offer the opt-out form to the parent
12 or guardian upon administration of the vaccination and the
13 parent or guardian may execute the opt-out form for the health
14 care practitioner or entity at that time, return it at a later
15 date, or submit the form directly to the department. If the form
16 is delivered to the health care practitioner or entity, the
17 practitioner or entity must submit the form to the department.
18 Any records or identifying information pertaining to the child
19 must be removed from The decision to not participate in the
20 immunization registry must be noted in the registry if the
21 parent or guardian has refused to have his or her child included
22 in the immunization registry.
23 3. A college or university student, from 18 years of age to
24 23 years of age, who obtains a vaccination from a Florida
25 college or university student health center may refuse to be
26 included in the immunization registry by signing an opt-out form
27 obtained from the department or from the Florida college or
28 university student health center which indicates that the
29 student does not wish to be included in the immunization
30 registry. The opt-out form must be provided to the department
31 and to the health care practitioner upon the administration of
