Section 467.016, Florida Statutes, is amended to 6 read 7 467.016 Informed consent.—The department shall develop a 8 uniform client informed consentinformed-consentform to be used 9 by the midwife to obtain the client’s consent for the provision 10 of midwifery services and to inform the client of: 11 (1) The qualifications of a licensed midwife;and12 (2) The nature and risk of the procedures to be used by a 13 licensed midwife;and to obtain the client’s consent for the14provision of midwifery services15 (3) The option to obtain medical care from a licensed 16 physician or certified nurse midwife; 17 (4) The hospital admitting privileges of the licensed 18 midwife; and 19 (5) The professional liability insurance status of the 20 licensed midwife. 21 Section 2. This act shall take effect July 1, 2020. 22 23 ================= T I T L E A M E N D M E N T ================ 24 And the title is amended as follows: 25 Delete everything before the enacting clause 26 and insert: 27 A bill to be entitled 28 An act relating to informed consent for midwifery 29 services; amending s. 467.016, F.S.; requiring a 30 licensed midwife to inform the client of specified 31 information; providing an effective date.
