    1         Senate Amendment to Amendment (694892) 
    2  
    3         Delete lines 16 - 17
    4  and insert:
    5  the parent’s or legal guardian’s written documentation
    6  establishing that he or she is the minor’s lawful parent or
    7  legal guardian must be attached to the notarized document.
