The purchaser must 6 furnish to the selling dealer an affidavit stating that the 7 impact-resistant windows, impact-resistant doors, or impact 8 resistant garage doors to be exempted are for the exclusive use 9 designated herein and must include the name of the owner making 10 the purchase and the address of the residential real property 11 where the items will be installed. Any person furnishing a false 12 affidavit to such effect for the purpose of evading payment of 13 any tax imposed under chapter 212, Florida Statutes, is subject 14 to the penalties set forth in s. 212.085, Florida Statutes, and 15 as otherwise provided by law. 16 17 ================= T I T L E A M E N D M E N T ================ 18 And the title is amended as follows: 19 Delete line 7 20 and insert: 21 term “impact-resistant”; requiring purchasers of 22 certain items to furnish a specified affidavit and 23 information to the selling dealer; providing a 24 criminal penalty for furnishing a false affidavit with 25 certain intent; specifying locations where
