Senator Brandes moved the following:

4 and insert:
5 (c)1. If the applicant has submitted the last four digits
6 of his or her social security number, the online voter
7 registration system must verify the last four digits of the
8 social security number in accordance with s. 97.053(6). The
9 applicant must also provide an electronic image of his or her
10 signature. The department shall adopt rules to authorize a
11 secure method for an applicant to upload or otherwise provide a
12 high-quality electronic image of his or her signature through
13 the online voter registration system.
14 2. If the last four digits of the applicant’s social
15 security number are verified pursuant to s. 97.053(6), the
16 online voter registration system shall transmit, using the
17 statewide voter registration system maintained pursuant to s.
18 98.035, the applicant’s registration application, along with the
19 electronic image of the applicant’s signature provided pursuant
20 to subparagraph 1., to the supervisor of elections. The
21 electronic image of the applicant’s signature satisfies the
22 signature requirement of s. 97.052(2)(q).
23 (d) If the applicant’s name and date of birth cannot be
24 verified by the records of the Department of Highway Safety and
25 Motor Vehicles, or if the last four digits of the applicant’s
26 social security number cannot be verified applicant indicated
27 that he or she has not been issued a Florida driver license or
28 Florida identification card, the online voter registration
29 system shall populate the applicant’s information into a
30 printable voter registration application pursuant to s.
31 97.052(2) and direct the applicant to print, sign, and date the
32 application and deliver the application to the supervisor of
33 elections for disposition pursuant to s. 97.073.
34 Section 3. Paragraph (a) of subsection (5) and subsection
35 (6) of section 97.053, Florida Statutes, are amended to read:
36 97.053 Acceptance of voter registration applications.—
37 (5)(a) A voter registration application is complete if it
38 contains the following information necessary to establish the
39 applicant’s eligibility pursuant to s. 97.041, including:
40 1. The applicant’s name.
41 2. The applicant’s address of legal residence, including a
42 distinguishing apartment, suite, lot, room, or dormitory room
43 number or other identifier, if appropriate. Failure to include a
44 distinguishing apartment, suite, lot, room, or dormitory room or
45 other identifier on a voter registration application does not
46 impact a voter’s eligibility to register to vote or cast a
47 ballot, and such an omission may not serve as the basis for a
48 challenge to a voter’s eligibility or reason to not count a
49 ballot.
50 3. The applicant’s date of birth.
51 4. A mark in the checkbox affirming that the applicant is a
52 citizen of the United States.
53 5.a. The applicant’s current and valid Florida driver
54 license number or the identification number from a Florida
55 identification card issued under s. 322.051, or
56 b. If the applicant has not been issued a current and valid
57 Florida driver license or a Florida identification card, the
58 last four digits of the applicant’s social security number.

60 In case an applicant has not been issued a current and valid
61 Florida driver license, Florida identification card, or social
62 security number, the applicant shall affirm this fact in the
63 manner prescribed in the uniform statewide voter registration
64 application.
65 6. A mark in the applicable checkbox affirming that the
66 applicant has not been convicted of a felony or that, if
67 convicted, has had his or her civil rights restored through
68 executive clemency, or has had his or her voting rights restored
69 pursuant to s. 4, Art. VI of the State Constitution.
70 7. A mark in the checkbox affirming that the applicant has
71 not been adjudicated mentally incapacitated with respect to
72 voting or that, if so adjudicated, has had his or her right to
73 vote restored.
74 8. The original signature or a digital signature
75 transmitted by the Department of Highway Safety and Motor
76 Vehicles of the applicant swearing or affirming under the
77 penalty for false swearing pursuant to s. 104.011 that the
78 information contained in the registration application is true
79 and subscribing to the oath required by s. 3, Art. VI of the
80 State Constitution and s. 97.051.
81 (6) A voter registration application, including an
82 application with a change in name, address, or party
83 affiliation, may be accepted as valid only after the department
84 has verified the authenticity or nonexistence of the driver
85 license number, the Florida identification card number, or the
86 last four digits of the social security number provided by the
87 applicant. If a completed voter registration application has
88 been received by the book-closing deadline but the driver
89 license number, the Florida identification card number, or the
90 last four digits of the social security number provided by the
91 applicant cannot be verified, the applicant shall be notified
92 that the number cannot be verified and that the applicant must
93 provide evidence to the supervisor sufficient to verify the
94 authenticity of the applicant’s driver license number, Florida
95 identification card number, or last four digits of the social
96 security number. If the applicant provides the necessary
97 evidence, the supervisor shall place the applicant’s name on the
98 registration rolls as an active voter. If the applicant has not
99 provided the necessary evidence or the number has not otherwise
100 been verified prior to the applicant presenting himself or
101 herself to vote, the applicant shall be provided a provisional
102 ballot. The provisional ballot shall be counted only if the
103 number is verified by the end of the canvassing period or if the
104 applicant presents evidence to the supervisor of elections
105 sufficient to verify the authenticity of the applicant’s driver
106 license number, Florida identification card number, or last four
107 digits of the social security number no later than 5 p.m. of the
108 second day following the election.
109 Section 4. Effective July 1, 2022, paragraph (a) of
110 subsection (5) of section 97.053, Florida Statutes, as amended
111 by section 3 of this act, is amended to read:
