The Committee on Judiciary (Hutson) recommended the following:
1 Senate Amendment (with title amendment)

3 Delete lines 37 - 51
4 and insert:
5 3.(c) The specific location of each alleged exposure for
6 each defendant.;
7 4.(d) The beginning and ending dates of each alleged
8 exposure as to each asbestos product or silica product for each
9 location at which exposure allegedly took place for the exposed
10 person plaintiff and each index person.;
11 5.(e) The occupation and name of the employer of the
12 exposed person and each index person at the time of each alleged
13 exposure.;
14 6.(f) The specific condition related to asbestos or silica
15 claimed to exist.; and
16 7.(g) Any supporting documentation of the condition claimed
17 to exist.
18 (b) The sworn information form is inadmissible in evidence
19 at trial and may not be relied upon by a witness, including an
20 expert witness, at trial.
