Health Insurance; amends definitions used in Fla. Comprehensive Health Association Act; revises criteria for eligibility for coverage from association; authorizes association to place individual with case manager who determines health care system or provider; requires insurers to be members of association & to be subject to assessments for operating expenses; revises coverage, benefits, covered expenses, premiums, & deductibles of assn., etc. Amends Ch. 627.
