Health Care Representatives; Provides for designation of health care representative; provides for designation of separate health care representative to consent to mental health treatment in certain circumstances; provides for effective date of designation; provides that written designation creates rebuttable presumption of clear & convincing evidence of principal's designation; provides suggested form, etc.
