Childhood Vaccinations [SPSC]; Authorizes a parent or guardian to object to immunizing a minor on philosophical grounds. Requires that the parent or guardian sign a form in order to refuse to vaccinate the minor. Provides requirements for the form. Requires that the parent or guardian periodically sign a new form in order to continue the exemption.
