Health Care Transition/Adolescents/Young Adults [SPSC]; Establishes an office within the Division of Children's Medical Services Network in the DOH to implement a health care transition program for adolescents and young adults who have special health care needs. Requires the DOH to enter into contracts, contingent upon appropriations, with local health and transition services programs. Specifies the services that may be offered by local health and transition services programs, etc.
