Project Description:

 

 

 

 

 

 

 

The development and implementation of a community-based neighborhood service center, which will promote the well being of children and families to increase parent's confidence and competence in their parenting abilities, focus on health prevention and child abuse prevention.The target population will be children in at-risk families, and/or children who have been neglected or abused.Services to be provided will be family support services, parenting education, mental health services including outpatient medical (individual, goup and family), health prevention (health screening, education, nutrition counseling, teenage pregnancy prevention, HIV/AIDS prevention, diabetes and hypertension education); pre-placement prevention services to help children at risk of foster care placement remain safely with their families.It will reduce the amount of funds currently needed for emergency and advance care (long-term and critical care).Educated parents and children will engage in better health practices and have a better functioning family.The family will become empowered to advocate and solve its problems.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

100% of youth to have required immunizations,reduction in child abuse and teen pregnancy, 70% of family participation in health education, 90% of children to enroll in Kidcare program, 70% of childern to show improved academic performance, 85% of families in apartment complex will participate
