Project Description:

 

 

 

 

 

 

 

This program is a collaborative effort by 5 local mental health agencies that provide comprehensive mental health services to adults who have had difficulty engaging in treatment or have recently been released from incarceration.The partners include Directions for Mental Health, Inc.; Personal Enrichment through Mental Health Services, Inc.; Gulf Coast Community Care; The Harbor Behavioral Healthcare Institute, Inc.; and Suncoast Center for Community Mental Health, Inc., as the lead agency.This program assists clients by linking them directly with services and teaches the clients how to access and follow-up with traditional outpatient programs.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

The effectiveness of the program will be gauged by evaluating the clients' improvement in functioning and by reducing recidivism.There should be a decrease in the need to pursue services through more expensive providers.
