Project Description:

This project provides preventative healthcare screenings and services to frail elders in 16,500 housing units and in more than 50 faith-based locations.

Measurable Outcome Anticipated:

Program participants will be significantly less likely to require stat-funded hospitalizations and/or nursing home care.� On the basis of 5,000 elders served, we anticipate 20% to 70% reduction in fall-related injuries and significant state savings.
