Project Description: |
We have built our Tri-Care Model around the stages of the Alzheimer's disease process and now are designing and planning to build a facility to meet each stage of the disease, i.e. Early, Middle adn Late Stages. The central, or hub is our 8,028 square foot Special Care Adult Health Center. This center will house two - 24 participant - Multi-Therapeutic Intervention stations, with an observation room for teaching and educational purposes. It will include a shower, respite beds for day activities, barber and beauty shop, housekeeping, kitchen, and a garden type walkway. The center will also have offices for the administration, program manager, family counceling and in-service training center, MSW (Caregiver Outreach Program), health care coordinator plus a large dining area, which can be used for special programs. Our Multi-Therapeutic Intervention Program will be focusing on a more personal and individualized program that will meet the basic human needs of everyone we serve. Therapy programs have been disigned and evaluated to promote the emotional, mental, social and physical growth of each client as well as meeting basic human need, need for dignity, need to experience something new and need for creativity. The Embers will strive to keep loved ones in their home environment for a longer period of time - and the cost of Day Care is less than half that of nursing homes. THESE REQUESTED MONIES WILL SECURE A $1,500,000 CONSTRUCTION LOAN. |

Measurable Outcome Anticipated: |
With the implementation of our programs for caregiver outreach and our mulit-therapeutic intervention, we hope to solve many of the challenges faced by the clients. This will in turn lengthen time spent in home care and reduce the stress factor and depression with caregivers, to stop early placement into long-term care by maintaining the family unity. Because of excessive stress and lack of proper support the participant is placed in a nursing home to enable the caregiver to escape. We are designing our program to eliminate or reduce this pratice, which has placed excessive financial burden on hte Medicaid system. With preliminary data projected over a ten-year period, using 2004 cost, we estimate we could see overall savings of about 46% in overall health care cost. |
