Project Description:
The continued implementationof an aftercare and residential program for Dual Diagnosed (Mental Illness and Subtance Abuse) individuals released from Correction Facilities. Funds now expend on rearrest and reimprisonment for these individuals can be reduced and thsi will assist in reduction of criminal acts that are drug related and its consequences.

Measurable Outcome Anticipated:
70% of severely mentally ill participants will start to receive mental health treatment voluntarily within six months; 30% of substance abuse participants will successfully complete substance abuse treatment within one year; 40% of participants without income/entitlement will apply for SSI withing 60 days; 65% of participants without life skills will voluntarily enroll in day treatment and start day treatment services with sixty days; 80% of participants will maintain permanent residence fora at least one year; 40% of participants will enroll in employability skills training withing 90 days of admission.
