Project Description:
Maintaining 7 dimentia-specific day service programs within neighborhoods 15 minutes from a caregiver's home in Palm Beach and Martin Counties Programs are specifically designed for Alzheimer's Disease and related disorder patients that qualify under Alzheimer's Disease Initiative(ADI)program administered by DOEA. Funds will be used for patients who cannot afford daycare expenses and for case management duties in accordance with ADI guidelines. Funds will directly support expenses, including salaries, indirect expenses required to operate the programs.

Measurable Outcome Anticipated:
600 patients will be served and receive dementia-specific care through the 7 daycare programs. All participants will be registered with the Nat'l Alzheimer's Association Safe Return Bracelet Program; receive daily personal care, medically monitored daily nurses who specialize in dementia-specific behavior and health care and receive therapeutic activities. Program will delay nursing home placement by 2 to 4 years, entirely preventing it for 65% of the programs patients. Family members will receive education, have access to weekly support group and have information and resources about the disease through ACC's information and referral service. Patients and community will have access to options that are humane and dignified which is a hallmark service for Palm Beach and Martin Counties.
