| Project Description: |
| Mental Health Care, Inc., proposes triage and emergency crisis response services to children and their families in the Suncoast Region. The staff will be available to Child Protective Investigators 24/7 for triage, assessment, case management, and therapeutic services. Master's level therapists and bachelor's level case managers will assist families for a period of 30 days in order to stabilize crisis situations and minimize the number of "out of home placements." Families will be linked to community resources prior to the termination of the program's services to ensure follow up and continuous care. |

| Measurable Outcome Anticipated: |
| The program's goal would be to prevent a minimum of 50% of all cases receiving the client services specified in item #7 from being removed from the home. Additionally, Parental Accentance Scale scores would improve by a minimum of 10 points for at least 50% of the families served. |
