This porposal supports and enhances the existing program, with the inclusion/expansion of a receiving center, operated by local community mental health agencies. Services include: immediate evaluation and assessment; psychosocial assessment; individual & family therapy; psychiatric evaluation; medication therapy; assistance in locating housing; assistance in meeting parole or probation requirements; advocacy in court proceedings; care coordination; group therapy. Outreach is conducted in many locations (including jails, hospitals, homeless shelters, and halfway houses).
