Project Title: Elderly Meals Program Date Submitted 1/16/2004 2:16:27 PM

Sponsors: Diaz de la Portilla

Statewide Interest:
Should LHANC be granted this request, approximately 400 elderly homebound clients will be able to receive a hot meal daily, as well as telephone reassurance and home visits from assigned LHANC staff on a regular basis. These home visits will also serve to identify other unmet needs clients/caregivers may have allowing for appropriate referrals to be made to other LHANC services (i.e.,caregiver support, respite care)or referrals to outside agencies for services that LHANC may not have available (i.e., personal care) thereby keeping the elderly living at home, albeit with some assistance and delaying or preventing their placement into nursing homes at a higher cost to the state.
