Should LHANC be granted this request, approximately 400 elderly homebound clients will be able to receive a hot meal daily, as well as telephone reassurance and home visits from assigned LHANC staff on a regular basis. These home visits will also serve to identify other unmet needs clients/caregivers may have allowing for appropriate referrals to be made to other LHANC services (i.e.,caregiver support, respite care)or referrals to outside agencies for services that LHANC may not have available (i.e., personal care) thereby keeping the elderly living at home, albeit with some assistance and delaying or preventing their placement into nursing homes at a higher cost to the state. |

Project Description: |
LHANC is requesting $475,000 in reccuring funds for the home delivered meals program. LHANC intends to provide approximately 104,625 home delivered meals to approximately 400 isolated, frail, homebound, elderly living at or below the poverty level and at-risk for malnutrition and early institutionalization with a hot meal. Participants will be removed from LHANC's waiting list for services, which currently has 1,200 elderly eligible to recieve services. |

Measurable Outcome Anticipated: |
Goal: Age in Place.
Objective: Help elders to live in the least restrictive and safe settings most appropriate. Outcome: 85% of original clients will remain living in the least restrictive setting -their own homes - one year after beginning to receive congregate or home delivered meals.
Output: Need # of clients who begin to receive services: also # of clients who remain receiving services at 1 year anniversary, excluding those who began services after original start. |
