| Project Description: |
| Provide quality community-based Elder Care Services, adult day care, meal preparation, health screenings, medical appointments, respite care, financial assistance coordinaion and referral, home repair, maintenance assessment and referral services for the frail, moderately handicapped and/or slightly disoriented elderly persons 60 years of age or over. The center will offer relief to family caregivers so that they can participate fully in the labor force.
This form is being used to ask for $700,000 to expand services to more families. The answer to question 10 is the expansion dollars requested. Question 13 has been answered none as no dollars have been appropriated.
The answer to question 11 will be the cost of the program if the additional sums are appropriated.
As no money has been appropriated, questions 15 and 16 are answered unknown as are 17 and 18. |
