| Project Description: |
| Establishment of a Parkinson's Action & Resource Center through which TCCP will emphasize empowerment of Parkinson's patients and Care Partners through information enabling them to make informed decisions about their care. A critical component of TCCP's mission is to provide information and guidance on accessing community-based and alternative resources that make it possible for People with Parkinson's to choose to safely stay in their own homes in addition to having access to support enabling them to share in the community, reduce social isolation, and ensure access to both volunteer and paid services. The goal to delay for as long as possible or altogether to prevent institutionalization due to the progressive deterioration in motor and cognitive function associated with Parkinson's.
