Project Description:
Establishment of a Parkinson's Action & Resource Center through which TCCP will emphasize empowerment of Parkinson's patients and Care Partners through information enabling them to make informed decisions about their care. A critical component of TCCP's mission is to provide information and guidance on accessing community-based and alternative resources that make it possible for People with Parkinson's to choose to safely stay in their own homes in addition to having access to support enabling them to share in the community, reduce social isolation, and ensure access to both volunteer and paid services. The goal to delay for as long as possible or altogether to prevent institutionalization due to the progressive deterioration in motor and cognitive function associated with Parkinson's.

Measurable Outcome Anticipated:
(1) Establishment of the Parkinson's Action & Resource Center by end of 3rd quarter of FY 2006. (2) Service delivered to minimum 350 peole with Parkinson's plus their Care Partners in Year 1, increasing by 1000 by Year 3. (3) Maintaining records on patients who go into Nursing Homes, reasons why the go into Nursing Homes, and the effectiveness of TCCP's programs in maintaining patients in their homes, where they are more comfortable and where they do not become a burden on the State of Florida.
