Statewide Interest:
The High Risk Nutritional Meals Project was designed to provide supplemental nutritious meals to elderly residents (at least 60 years or older, attend a congregate meal site, i.e. Adult Day Care and/or Senior Center, have been determined to have a nutritional score of 5.5 or higher as determined by the State of Florida Client Information Registration Tracking System Assessment and have income at or below the poverty level) who are assessed as "high risk" and/or in danger of malnutrition. The goal of the project is to reduce or delay the need for more expensive nursing home placement further burdening public healthcare, i.e. Medicaid.

Project Description:
Provides supplemental nutritional meals to elderly County residents who are assigned as "high-risk" and/ or in danger of malnutrition. The criteria for the High Risk Nutritional Meals Program is that participants attend a congregate meals site (i.e. Adult Day Care and/or Senior Center); be at least 60 years of age or older; have been determined to score 5.5 or higher as determined by the State of Florida, Client Information Registration Tracking System Assessment and have income at or below the poverty level. Priority is given to those who score the lowest in income category and highest in the nutritional risk-scoring category.
