The project has demonstrated community support as well as support from the Department of Health, and the Agency for Health Care Administration. |

| Project Description: |
| Provision of a comprehensive school health program to ensure that all children have access to quality health care services, including a medical home and behavioral health services through a health team in every school to improve health, health education, school environmental and school success. State funding is requested for start-up costs associated with the development of the Management Information System (MIS) and for other non-recurring costs such as health and medical supplies. |

| Measurable Outcome Anticipated: |
| Outcomes would be measured through data collection to determine the number of children linked to a medical home, the number of children receiving well-child checkups and screenings, a variety of health outcomes and the number of days of school missed, which should decrease and consequently school performance which should ultimately improve. |
