Project Description:
The four specialized day service sites will serve 412 Alzheimer's disease and related disorders patients and caregivers annually. Three service sites were established during 2004 thru 2006 and the fourth is planned to open in Riviera Beach for 2006/07. They operate five days a week, 7:30 AM to 5:30 PM. These hours of operation accommodate the 45% of our caregivers who work, allowing ample time during peak travel hours. There is and will be a patient/staff ratio of 1/5, with volunteers decreasing that ratio to approximately 1/3. Approximately 5 to 15 volunteers will be trained to work at each dementia-specific day service site. Therapeutic activities, stimulating exercises (mind and body) are provided during the day, along with nutritional meals, and medical monitoring by a qualified licensed nurse who can dispense medications, administer glucose testing, provide heart and cancer disease maintenance, hospice practices when appropriate and track the progression of the disease process, insuring stabilization and longevity within the various sites. In addition, disaster preparedness and hurricane planning for caregivers as well as patients is always a component of case management for those who participate in the day service sites. All dementia-specific day service sites operate in donated spaces which are handicap accessible and with clergy affiliations, but totally separate of their religious teaching. Caregiver support groups will meet weekly on the premises, with other therapeutic training and educational seminars scheduled. All funds will be used to provide services to Alzheimer's disease and related disorders patients and their caregivers. Recruitment and hiring of staff and volunteers for the Riviera Beach site will occur three months prior to the anticipated opening, in addition to purchasing specialized regulated equipment and furniture to accommodate the 70 patients served by that site. Alzheimer's Community Care provides access 24 hours a day, seven days a week to case management, information and resources.

Measurable Outcome Anticipated:
: To lower institutionalization of patients by two to four years; lower the risk of the mortality of caregivers from 63% to approximately 40% or lower; lower the risk of hospitalization for caregivers. Approximately 13% of patients annually (identified by our evidence-driven data) are at risk of discharge from the day service sites, however 87% will not enter a nursing home -- a savings of institutional dollars totaling $10,450,351.50 ($55,001.85 Medicaid x 190 patients) to caregivers, community and society.
