Project Description:
Provide comprehensive education programs and resources for effective advance care planning and Advance Directive utilization to healthcare professionals, legal professionals, estate planners,pre-need planning professionals, clergy of all denominations, social service professionals and the general public including adults of all ages.

Measurable Outcome Anticipated:
Significant increase in adult residents throughout the state of Florida with clearly documented Advance Care Directives based on their own ethical, cultural and spiritual beliefs and values resulting in better quality of end-of-life care, reduced healthcare costs and will alleviate family conflicts in making end-of-life medical treatment decisions.
