According to the 2000 National Demographic Study, the average age of Holocaust survivors is 75 years and 13% live in the south Florida area. Because Holocaust survivors suffer extreme problems when placed in Nursing Homes and assisted living facilities greater efforts need to be taken to allow them to stay at home. Thirty-five percent (35%) of the survivors reported that they were "just managing" compared to non-survivors response of 23%. South Palm Beach County, Florida is home to an estimated 5,100 survivors of the Holocaust. Holocaust survivors are much more likely to need additional help than their counterparts who did not experience their physical and mental torture. Because Holocaust survivors often suffer severe mental health problems when placed in Nursing Homes and assisted living facilities,they require more home healthcare services to enable them to remain at home. Survivors that did not require our assistance are becoming increasingly frail and unable to manage their activities of daily living as easily as before and those that are receiving our help will require a more intensive level of care as they age and their frailty increases.

Project Description:
Calls are received at our agency from Holocaust survivors in need of in home services. In response to that call a case manager is sent out to make a home visit. During the home visit an assessment is conducted to determine the type and scope of services needed. The resultant service plan is then forwarded to a vendor to provide the necessary services. The services provided include, personal care (bathing and dressing), housekeeping, meal preparation, laundry, shopping, and transportation to medical appointments. Housekeeping service is provided on average once a week. Personal care and other services are provided 2-3 times a week on average. The case manager visits the client at least once every three months to review and reevaluate the client's level of functioning and determines whether or not there is a need to make changes to the care plan. Clients are assessed based on the following factors: dressing, grooming, bathing, eating, transferring - includes getting out of a bed or chair, walking/mobility, stair climbing, toileting, bladder/bowel control, client incontinence, ability to use telephone, shopping, transportation ability, preparation of meals, laundry, light housekeeping, heavy chores, taking medication, and managing money. These items are all from the Department of Elder Affairs Comprehensive Assessment Form as well as the Short Assessment From. Additionally, clients are given an assessment of their mental health status with such questions as what year and month is it now? Finally, the client has an assessment of medical conditions, ranging from false teeth to a wheelchair. Most of the Holocaust Survivor population does not receive government means tested assistance of any kind. They do receive social security, Medicare and may receive modest reparations. Survivors, as is true of all elderly, generally require more assistance with age. In addition to home care and case management, survivors also require and receive mental health counseling, food pantry supplements, kosher meals, telephone reassurance and socialization activities.

Measurable Outcome Anticipated:
Holocaust survivors will receive the in home services that they need based on the results of the needs assessment performed on each client by the case manager. Clients will be surveyed after they begin to receive services to evaluate how effective the services are and measured to determing,Provision of the appropriate services based on the client's needs will at least delay if not eliminate the need for the client to enter an assisted living facility or nursing home. The specific measurement is whether or not the client receives the services as per the assessed needs, and whether or not the client continues to reside in the community once services are provided; the socialization helps older citizens stay productive, reduces depression.
